Job Overview:

RWJBarnabas Health is seeking a dedicated and experienced Gerontological Nurse Practitioner to join our healthcare team. In this pivotal role, you will provide comprehensive, patient-centered care to older adults, focusing on the prevention, diagnosis, and management of age-related health issues. Your expertise will be essential in guiding our patients through complex health challenges while promoting their overall well-being, working with other clinicians in the practice.

Certifications, and Licenses:

  • Master's degree in Nursing
  • Registered Professional Nurse with a current NJ state license
  • APN License
  • NP Certification
  • CDS and DEA
  • CPR certification

Required:

  • Experience and training in the field of geriatric care
  • 3-5 years of clinical experience in geriatric care or a related field

Preferred:

  • Strong knowledge of geriatric pharmacology and common age-related health conditions
  • Experience in a primary care or long-term care setting
  • Proficiency in electronic health record (EHR) systems
  • Active involvement in professional organizations related to gerontology
  • Expertise in comprehensive geriatric assessment tools and evidence-based interventions
  • Familiarity with telehealth technologies to enhance patient access and care delivery
  • Ability to utilize clinical decision support tools to inform practice
  • Exceptional communication and interpersonal skills to build rapport with patients and families
  • Empathy and compassion for older adults and their unique healthcare challenges
  • Strong critical thinking and problem-solving abilities to navigate complex clinical scenarios
  • Commitment to continuous professional development and staying current with gerontological research
  • Working with other office practitioners to ensure optimal care for patients
  • Flexibility providing supportive/collaborative care with other practitioners to optimize patient care and outcomes

Scheduling Requirements:

  • Full time
  • 40 hours
  • Monday-Friday 8:00-5:00

Essential Functions:

  • Conduct thorough assessments of geriatric patients, including physical exams, health histories, and psychosocial evaluations
  • Develop and implement individualized care plans addressing the unique needs of older adults
  • Prescribe medications and manage treatment regimens while monitoring for effectiveness and potential side effects
  • Coordinate care with interdisciplinary teams, including physicians, nurses, social workers, and therapists
  • Educate patients and families about disease management, health promotion, and preventive care strategies
  • Maintain accurate and comprehensive patient records in compliance with healthcare regulations
  • Participate in quality improvement initiatives and contribute to the advancement of gerontological nursing practices within the organization
  • Working collaboratively with Geriatricians to assist in optimizing patient care and outcomes
