Description MaineHealth Corporate Management/Leadership The Manager (RN) - Clinical Care Navigation, Virtual Care will lead and support an innovative Care Navigation Program designed to improve the healthcare experience, reduce costs, and minimize care fragmentation. This position will oversee a multidisciplinary team of RNs and non-RNs, ensuring high-quality patient care and operational excellence while dividing time between team leadership and direct patient navigation. The role will coordinate care and services for selected patient populations across the continuum of illness, promote effective utilization and monitoring of healthcare resources, and proactively identify and address barriers to treatment, health, wellness, and outcomes. Serving as a vital link between patients, healthcare teams, payers, and community resources, the Manager will foster collaboration to achieve optimal patient-centered, clinical, and resource outcomes while actively participating in mentorship, training, and process improvement within the team. This position is based in Portland (Currently at our Free Street location) and enjoys a hybrid schedule (with weekly onsite expectations). Summary: The Manager - Clinical Care Navigation will be responsible for overseeing a multidisciplinary team of RNs and non-RNs, ensuring high-quality patient care and operational excellence. This individual will split their time between managing the care navigation team and directly engaging in patient care navigation, guiding patients through their healthcare journey with compassion and expertise.
