The Manager (RN) - Clinical Care Navigation, Virtual Care will lead and support an innovative Care Navigation Program designed to improve the healthcare experience, reduce costs, and minimize care fragmentation. This position will oversee a multidisciplinary team of RNs and non-RNs, ensuring high-quality patient care and operational excellence while dividing time between team leadership and direct patient navigation. The role will coordinate care and services for selected patient populations across the continuum of illness, promote effective utilization and monitoring of healthcare resources, and proactively identify and address barriers to treatment, health, wellness, and outcomes. Serving as a vital link between patients, healthcare teams, payers, and community resources, the Manager will foster collaboration to achieve optimal patient-centered, clinical, and resource outcomes while actively participating in mentorship, training, and process improvement within the team.

This position is based in Portland (Currently at our Free Street location) and enjoys a hybrid schedule (with weekly onsite expectations).

Summary:

The Manager - Clinical Care Navigation will be responsible for overseeing a multidisciplinary team of RNs and non-RNs, ensuring high-quality patient care and operational excellence. This individual will split their time between managing the care navigation team and directly engaging in patient care navigation, guiding patients through their healthcare journey with compassion and expertise.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: BSN or active enrollment in BSN program.
2. License/Certifications: Current State of Maine RN license required. Current BLS Certification required or must obtain within 30 days of start date.
3. Experience: Three years of Registered Nursing experience required. Leadership experienced preferred.
4. Additional Skills/Requirements Required: Prior care management/care coordination experience.
5. Additional Skills/Requirements Preferred: Effective communication skills. Critical thinking and problem-solving skills. Program development experience.
