Job Title: Case Coordinator - Social Worker Department: Case Coordination General Summary: As a Case Coordinator - Social Worker II, you will play a pivotal role in enhancing patient care by evaluating the timeliness and appropriateness of services, and expertly coordinating discharge planning to ensure seamless transitions to post-hospital care. This position also involves providing insightful case reviews and recommendations for patients in the Renaissance Unit, making a meaningful impact on their recovery journey. Key Responsibilities:
- Evaluate patient care by reviewing medical records and collaborating with care providers to align with industry standards.
- Identify and resolve delays in patient care by effectively negotiating with internal and external providers.
- Assess complex discharge planning needs and collaboratively develop personalized post-hospital care plans.
- Facilitate multidisciplinary meetings to address complex cases and enhance care strategies.
- Attend patient-centered multidisciplinary meetings to foster interdisciplinary collaboration and achieve positive health outcomes.
- Document all case coordination activities meticulously in Epic, ensuring comprehensive care records.
- Manage discharge planning orders, ensuring accurate and clear communication with care teams.
- Act as a liaison to payers and external providers, coordinating essential post-hospital services.
- Facilitate effective information transfer among healthcare professionals for smooth transitions across care levels.
- Refer patients with unique age-specific needs to appropriate community services.
- Identify staff learning opportunities related to case coordination, conducting engaging educational sessions.
- Analyze data on practice patterns to uncover trends and drive continuous improvement.
- Uphold infection control and safety practices in all aspects of work.
- Perform additional duties as required to support team and organizational goals.

Qualifications:
- Licensed Social Worker in D.C. with a Master's degree in Social Work or a related field.
- 2-5 years of social work experience in healthcare, including expertise in case coordination and inpatient utilization management.
- Current CPR certification.
Salary Range: Minimum 0.00/hour - Maximum 52.89/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
