Benchmark is hiring for a Part Time Licensed Practical Nurse (LPN) to serve as a member of the Community Action Treatment (CAT) team. The focus of the CAT program is to provide time-limited home and community based therapeutic, supportive services on a scheduled and/or crisis basis to support youth and their families with behavioral health disorders, substance use disorders and/or developmental disabilities that have been identified as being at risk of out of home placement, either due to treatment in a more restrictive setting or removal by the Department of Family and Children Services (DFCS).

In Community based position - Travel required throughout region 1 of Georgia

The part time Licensed Practical Nurse (LPN) is responsible for the health and safety of individuals receiving services through CAT. The LPN functions within the limits of their license and educational preparation, as well as function effectively with other members of the care team. The nurse will utilize the nursing process under the clinical supervision of a psychiatrist or psychiatric APRN to promote and maintain health, prevent disease and other potential disabilities in order for the individual to obtain optimal health. Must be willing to flex schedule accordingly to provide services, supports, care and treatment to individuals as required.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Work with the Community Action Treatment team to provide direct care to individuals receiving services through the program.
  • Uses positive behavioral support strategies as described in behavior support plans or behavioral guidelines.
  • Care includes providing medical treatments, administering medication and giving injections as ordered by a physician or nurse practitioner licensed in the State of Georgia
  • Ensure that an inventory of medications is safely and securely maintained.
  • Measures and records individual's vital signs, such as height, weight, temperature, blood pressure, pulse and respiration, blood sugars etc.
  • Observes and interacts with youth and their families in a therapeutic manner consistent with education, training and program parameters.
  • Documents and reports any changes in individual's progress, conditions, such as adverse reactions to medication or treatment, changes in behavior, etc. Notifies appropriate treatment team members and discusses any necessary action.
  • Assist with the implementation of high intensity services such as screening, evaluation and assessment within the limits of their license.
  • Facilitate individual transition to other supports and/or treatment.
  • Enhance communication among staff to promote high quality care.
  • Review documentation for ethical and lawful billing and business practices.
  • Comply with all standards to assure the health and safety of all staff and individuals that we serve.
  • Must report any suspected abuse, neglect or exploitation to supervisor or department head
  • Attend all scheduled training and staff meetings.
  • Perform other duties as assigned.
  • Plan and participate directly in recreational, therapeutic, and training activities of the individuals.
  • Report to work as scheduled.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of DFCS and DBHDD Community Provider standards and organization policies and procedure.
  • Knowledge of Person-centered care and support for individuals with behavioral health disorders.
  • Skillful in use of computers & general office equipment.
  • Skillful in supervision of staff from diverse educational and cultural backgrounds.
  • Skillful in interpersonal communication.
  • Ability to oversee the services, supports, care and treatment provided to individuals in the crisis home.
  • Ability to direct, monitor, assess, and train staff in healthcare services.
  • Ability to assist in the formulation of the individual service plan or individual recovery plan.

QUALIFICATIONS:

Minimum:

  • Valid unrestricted Georgia Nursing License to practice nursing in the state of Georgia.
  • Certification and ongoing training in crisis intervention curriculum
  • Valid CPR and First Aid Certification.
  • Experience working with individuals with behavioral health disorders, substance use disorders and/or intellectual and/or developmental disabilities.
  • Valid Driver's License and auto insurance.
  • Must be computer literate.

Preferred:

  • Minimum of two years' experience working with youth with behavioral health disorders, substance use disorders and/or intellectual and/or developmental disabilities.
  • Two years' work experience as an LPN working with children and adolescents in an outpatient clinic/community-based setting.
  • Experience with both medical and behavioral crises.
