Overview This position is based in Lakeland, FL and requires travel within the Polk County territory. It is PRN 10 hours/Week Flex Schedule. Are you looking for a rewarding career in hospice? Technology Used Hospice RNs utilize and leverage: * HCHB = Proprietary Real-time database * Enclara Pharmacy solutions with 24/7 access * IPrescribe - Efficiencies with medication management * Forcura - Document management system * Medline - Supply ordering system * TalentEDGE - Amedisys' premier learning center * EBSCO - Nursing Resource Center for CEUs * Tablet with voice dictation and mileage tracking * Outlook added to Teams, Chat, Yammer Orientation, Onboarding & Training You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Responsibilities Performs initial nursing assessment on new admissions. Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff. Regularly communicates patient progress to the clinical manager, physician, and care team. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Educates families on patient condition, needs and care. Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned. Qualifications Current RN license, specific to the state(s) you are assigned to work. 2+ years of clinical RN experience in acute healthcare setting(s) - REQUIRED.
