POSITION SUMMARY: The Boston Medical Center Geriatric Services, Home Care Program provides primary medical care and intensive nursing case management to homebound frail elders living within the City of Boston. The Nurse Care Manager position is a unique one requiring a variety of nursing skills in order to effectively provide nursing case management for a panel of complex, high risk, nursing home level older adults. Position: Nurse Care Manager Department: Geriatrics Schedule: 40 Hours (Days) ESSENTIAL RESPONSIBILITIES / DUTIES: Performs a variety of high-level nursing care management services through the development and use of geriatric nursing practice standards. Activities include conducting a comprehensive geriatric nursing assessment, developing and reassessing a plan of care which addresses the physical and psycho-social needs of the older adult; triaging day to day clinical issues; and providing patient/family education via telephone and home visits as well as coordinating care and services across various sites of care. Ensures that desired patient outcomes are achieved and that variances from established best practices are evaluated and addressed as necessary. JOB REQUIREMENTS EDUCATION:

Compensation Range: $104,000.00- $151,000.00This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
