*Job Title* Care Coordinator
*Summary* Experienced Care Coordinator aligned to a specific region who will manage benefit coordination, support access to prescribed therapies, and serve as the primary liaison between patients, healthcare providers, payers, and internal stakeholders. *Key Responsibilities* * Serve as an expert resource on benefit coordination, program enrollment, affordability support, and access pathways for prescribed therapies. * Advocate for patients on eligibility, enrollment, and affordability, and assist providers with program-related inquiries. * Build and maintain trusting relationships and ongoing rapport with healthcare providers and patients. * Act as the primary point of contact for providers, delivering detailed program and patient information and supporting relationship development. * Interpret and explain payer benefits across private/commercial and government payers, including Medicare, Medicaid, VA, and DOD, with strong expertise in Medicare Part B. * Navigate payer processes and resolve coverage barriers by asking targeted questions and escalating when appropriate. * Serve as an assigned liaison to client contacts and regional sales representatives. * Maintain accurate records in compliance with program standards and applicable regulations. * Follow program SOPs, call guides, and escalation procedures for complex cases. * Coordinate between Program Management, internal teams, and healthcare providers to communicate patient status, prescriber feedback, and program effectiveness. * Assess clinical and administrative situations and intervene to achieve timely results. * Uphold strict confidentiality, privacy, and professional ethics. * Contribute to team morale through a positive attitude and strong work ethic; use appropriate resources for conflict resolution. *Required Skills* * Prior authorization management * Pharmacy and medication access processes * Medicare (including Medicare Part B) and Medicaid knowledge * Insurance verification and reimbursement procedures * Care coordination and case management * Insurance billing and claims basics * Medical terminology and patient registration * Data entry and call center/customer service skills * Proficiency with Microsoft Office and common technology platforms *Preferred Qualifications* * Experience interacting directly with patients and providers. * Demonstrated understanding of insurance reimbursement and reverification processes. * Familiarity with Free Goods programs and manufacturer support offerings. * Background in both medical and pharmacy benefits. * Strong attention to detail and commitment to quality outcomes. *Experience Level* 2 years of relevant experience. *Pay and Benefits* The pay range for this position is $24.00 - $24.00/hr.
