Behavioral Health Case Manager - Masters Level
The Mercyhealth Behavioral Health In-Patient Unit is dedicated to serving patients who are suffering from acute mental illnesses and medically monitor withdrawal. There are between six to twelve patients within the unit at one time and staffing is appropriately aligned. This team demonstrates their passion for making lives better by providing care in a safe environment through assessment of needs, individual and group intervention, development of trusting rapport, de-escalation as needed and development of a discharge plan. The Behavioral Health Case Manager MS works with the behavioral health multidisciplinary team to provide care for the Responsibilities Job Duties: * Works with patients on the inpatient unit, emergency room,medical floors and day treatment programs. * Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies. * Assesses treatment planning and progress. * Prepares treatment plan and goals for patients. * Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy. * Provides y psychoeducational groups. * Arranges for disposition and follow up recommendations. Requirements: Master's Degree in a mental health, behavioral health field with documented clinical course work. Experience or field study experience in mental health site. Working toward licensure is preferred. Approved CPR/BLS within 90 days after hire.
