Under limited supervision, functions as a consultant, educator, trainer, expert practitioner for the Care Management team (Social Workers and Nurse Case Managers). Can provide clinical supervision for Graduate Social Workers if needed. Assists with specific duties assigned by departmental leadership such as scheduling, staffing, assignments, orienting new staff, complex case reviews, assist with team meetings, and various short-term projects as needed. This position will still manage a direct patient caseload as determined by departmental needs. Serves as an organizational resource to ensure evidence-based practice, and to facilitate optimal patient care within GBMC. Provides services such as psychosocial assessment, complex discharge planning, coordination of services, resource referral, support group facilitation and consultation. Participates in interdisciplinary collaboration with RN Care Managers, Physicians, Residents, and the rest of the multi-disciplinary team. Collaborates with service line team members to distribute patient case load.

- Knowledge of various social, home care, extended care, hospice, government programs, commercial insurance and community services

Licensures, Certifications
- Licensed in Maryland as an LCSW or LCSW-C
- Board Certified to supervisor graduate social workers upon hire or by end of 90-day probationary period
Principal Duties and Responsibilities
- Interviews patients, family members and significant others to obtain a relevant psychosocial assessment and to assess current/potential needs.
- Develops and implements a plan of care appropriate to identified problems. Shares resultant findings with the appropriate health care team members, physicians, and payer source.
- Orchestrates discharge planning for patients, considering insurance coverage, home care, auxiliary care, etc., personal resources, support systems, and level of post acute placement appropriateness.
- Provides complex social work services including individual, family and group modalities and other recognized psychosocial therapies in assisting patients and family members.
- Provides interpretation of the patient's, significant others, and family members' behavior as needed. Offers suggestions for modifying behavior by recommending appropriate resources.
- Provides and facilitates completion of appropriate documentation for patient records in a timely manner.
- Identifies, plans, modifies, and coordinates patient discharge needs, making appropriate referrals, and utilizing available community resources as needed for implementation. Formulates a plan that addresses the continuing needs of the patient following discharge from the acute care setting.
- Provides as well as attends educational in-services to the rest of the interdisciplinary staff.
- Actively participates in problem solving to enhance the most efficient and appropriate discharge plan.
- Assists other departments within GBMC needing Social Work intervention on an as needed basis during unforeseen staffing shortages.
- Completes Readmission Interview consistently for appropriate admissions.
- Provides necessary services for effective post discharge placement in another facility, by obtaining necessary pre-authorization, transportation, and contacting (if necessary) the proposed facility regarding clinical information and services required.
- Assists in the orientation process of new team members, including curriculum and competency development and design of preceptor practicum.
- Communicates and delineates policy, procedures and protocols for patients and families within the clinical social work arena.
- Ensures the establishment, interpretation and maintenance of standards of practice within area of expertise.
- Provides educational programs to the interdisciplinary team based on educational needs assessment and quality data.
- Acts as resident clinical consultant, providing clinical guidance and mentoring.
- Acts as a educator, trainer to Care Management team members

Physical Requirements
- Ability to move quickly about the hospital to all units as needed. Able to concentrate on details in a hectic environment.
Working Conditions
- Work is performed inside the organization's offices, patient care units, or in patient rooms. Job attendance is required during all types of weather conditions.
- Must carry on-call phone for evening and weekend call on a rotating basis with ability to come in to work if necessary during time off.

Pay Range $60,150.49 - $102,255.84
Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
