Your Impact As Manager, Care Management you are an instrumental leader within the Care Management team. You manage the day-to-day activities of the team, ensuring all objectives meet and exceed program standards.
Requirements
Key Responsibilities
- Ensure the Care Management team operates effectively and efficiently within the guidelines of required Health Home core services.
- Ensure Care Management operates at maximum capacity to ensure as many members as possible have access to the services as well as meeting our fiscal projections.
- Provide individual and group supervision to Care Coordination staff, using your expertise to develop their talent.
- Enhance established workflows and protocols, assess on-going effectiveness, and make recommendations for improvements, as needed.
- Lead and facilitate case conferences and discharge planning meetings to determine if a member requires an alternate level of care or is appropriate for disenrollment.
- Ensure the Care Management team addresses barriers to health care, including but not limited to financial management, housing, transportation, social support, and understanding of health conditions.
- Manage Care Coordination staff in the provision of crisis intervention services; review crisis management plans and revisions of care plans as appropriate.
- Build effective and efficient relationships with medical, psychiatric and other care team members, as well as clubhouse staff, health insurance, and Managed Care Organizations.
- Maintain all EHR and care management database systems.
- Conduct quality assurance audits and activities to assess the effectiveness of services delivered. Ensure member charts and documentation follow Health Home policies and agreements. Approve documentation, assessments, and plans of care. Review team member's caseload and ensure all program documentation is completed and submitted in a timely manner.
- Ensure Care Management team adheres to compliance protocols and procedures set forth by lead Health Home.
- Carry a small caseload of HARP and non-HARP members and assume responsibility for care coordination staff caseload as needed.
- Recognize the member as a contributing member of the care team through social practice, motivational interviewing techniques, continuous transference and reinforcement of self-management and self-efficacy skills.
- Utilize approved Health Home assessment tools and Health Information Technology (HIT) to prepare initial and ongoing clinical and psychosocial assessments of service needs of identified members. Identify level of need and tailor care plan accordingly.
- Respond to in-patient and emergency admission alerts and contact medical providers/hospitals for admission/discharge information.
- Perform required assessments and screenings using Health Information Technology (HIT) dashboards; monitor member progress and produce/maintain detailed, accurate and timely case notes.
- Be instrumental and contribute to the continued effort toward integrated care services within the Fountain House care systems models.
- Educate Community System of Care within Fountain House to ensure all have an understanding of care management.
- Maintain compliance with all company policies and procedures.

What you bring
- Master's Degree in a behavioral healthcare related field such as Social Work, Mental Health Counseling, or Psychiatric Rehabilitation.
- 5 years of direct care experience in the mental health field.
- 3 years of supervisory experience.
