Key Responsibilities
* Attends daily Home Hospital team huddles to review patient status. * Collects relevant clinical information to identify eligible patients within the catchment area. * Collaborates with the ED and Hospital Care teams to support patient identification and transfer to MGB Home Hospital. * Coordinates with the Home Hospital Admitting Provider and Case Manager for patient eligibility and transfer logistics. * Obtains patient consent for transfer to Home Hospital. * Communicates with Administrative Coordinators and Managers regarding transfer processes. * Maintains real-time awareness of Home Hospital capacity and capabilities. * Supports patient flow and placement to meet Home Hospital census goals. * Applies critical thinking and sound judgment aligned with organizational objectives. * Acts as a role model, coach, and facilitator promoting teamwork, respect, and collaboration. * Demonstrates effective communication that fosters patient-centered care, relationship-building, and professionalism. 36 hour position- 12 hour shifts The position to which you are applying is represented by a collective bargaining unit,Massachusetts Nurses Association Requirements * Current, active RN licensure in the state of Massachusetts * 3+ years of experience working in an emergency department, home hospital, and/or inpatient setting The ideal candidate profile is a proven, thorough RN who is confident in their clinical abilities and is comfortable working not only autonomously, but also as part of a team.
