Job Announcement

The Fairfax County Health Department is a progressive public health leader serving 1.2 million residents from diverse ethnic, cultural and economic backgrounds. This position works as a part of our Tuberculosis (TB) Program team. Plays a vital role in delivering comprehensive treatment and support services to clients diagnosed with active TB disease or latent TB infection. Under the supervision of the TB Program leadership and a Public Health Nurse, this position will:

  • Administer Directly Observed Therapy (DOT) though in-person home visits or via approved video platforms to ensure consistent medication adherence on a daily or intermittent schedule as directed by the physician.
  • Act as a liaison between clients and the TB care team, including Public Health Nurse care managers, the TB Program Manager and Assistant Managers, and the TB Physician Consultant, to support coordinated case management and timely communication.
  • Assist with enforcing TB education and the client's individualized plan of care.
  • Facilitate client access to health and human services resources as needed.
  • Participate in community outreach and contact investigations under the director of the TB nursing and medical staff.
  • Provide education, support, and resources to clients to encourage treatment completion and improved health outcomes.
  • Maintain accurate and timely documentation of all encounters in accordance with program guidelines using the Fairfax County Health Department's electronic medical record system.

Note: This is a 30 hour/week merit position.

Required Knowledge Skills and Abilities

Knowledge of the basic principles of human behavior;
Knowledge of word processing and data management programs (Excel, survey software)
Ability to communicate effectively to clients;
Ability to organize information and records;
Ability to communicate in written format.

Employment Standards

MINIMUM QUALIFICATIONS: Any combination of education, experience and training equivalent to graduation from high school or G.E.D. issued by state department of education; plus three years of work experience, including two years in some area of human services (related two-year [associate] degree may be substituted for the required human service experience).
CERTIFICATES AND LICENSES REQUIRED:

  • Valid driver's license
  • AED (Required within 60 days)
  • Cardiopulmonary Resuscitation - CPR (Required within 60 days)

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)

Additional Work Schedule Requirements: Flexible dependent on department needs. May occasionally start as early as 7:00 AM or work as late as 9:30 PM based on service demands and occasionally on weekends. May be assigned to work at various sites.

PREFERRED QUALIFICATIONS:
  • Experience working in local public health settings or community health settings.
  • Demonstrated ability to deliver human services to diverse client populations in home, office, and community settings.
  • A proven ability to collaborate effectively within a multidisciplinary team, while upholding the highest standard of integrity, confidentiality, and professionalism.
  • Excellent problem-solving abilities, verbal and written skills, and interpersonal skills to engage successfully with individuals from diverse backgrounds.
  • Proficiency in Microsoft Office Suite, Outlook calendar, and other software applications is required.
  • Experience with electronic databases or electronic health record systems is highly preferred.

PHYSICAL REQUIREMENTS:
Job is generally sedentary in nature, and requires walking, standing, sitting (for long periods of time), and kneeling, reaching, bending, climbing stairs; may be required to lift or carry up to 25 lbs. as required for the position. Uses hands to grasp, handle, or feel. Visual acuity is required to read data from paper and on a computer monitor or other electronic device; ability to operate keyboard driven equipment and computer and use of touchscreen. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. Ability to drive a motor vehicle and maintain a valid driver's license; travel to various locations throughout the county. Work is generally performed in an office environment; however, this position is primarily required to perform duties outside of a typical office setting to conduct home visits and/or community outreach, including outdoor settings in all weather conditions and night-time hours. All duties may be performed with or without reasonable accommodations.
