Job Summary and Responsibilities

The purpose of this position is to complete screening reviews of referred individuals with possible Serious mental illness to gather all relevant information to determine the status of an LII PASRR referral for individuals admitting Medicaid Skilled Nursing facilities.

    • Monitor the internal CSR call queue to provide support and assistance for inbound calls when required.
    • Contact facilities to verify individual status, request medical records, and manage PASRR activities to ensure timely processing of referrals.
    • Maintain thorough documentation of all activities within the state system and in the Acentra's internal tracking System, ensuring compliance with contractual timeframes to process incoming referrals.
    • Manage, transfer, and coordinate incoming PASRR-related calls.
    • The schedule includes weekend shifts and hours from 8:30 AM to 5:30 PM (PDT), including some holidays.
    • Develops and maintains working knowledge of internal policies, procedures, and services (both departmental and operational)
    • Utilizes automated systems to log in and retrieve information. Performs accurate and timely screenings and data entry
    • Working knowledge of clinical criteria/risk factors for possible serious mental illness.
    • Computer proficiency in Microsoft Excel, Word and Outlook.
    • Ability to utilize computer equipment and web-based software to conduct work.
    • Ability to interact with team members as needed to support necessary workflows
    • Ability to communicate effectively to individuals and groups through spoken, written and electronic media.
    • Ability to attend to detail, effectively prioritize and execute tasks in a timely manner
    • Read, understand, and adhere to all corporate policies including policies related to HIPAA and itPrivacy and Security Rules.

Qualifications

  • High school diploma required
  • Bachelor's Degree in a health care related field preferred.
  • Willing to work Friday-Tuesday 8:30am-5:30pm(PDT) with some holidays (Wed. & Thurs. off)
  • Ability to identify and resolve problems in a timely manner, gather and analyze information skillfully; work well in group problem solving situations and maintain a calm demeanor in difficult situations.
  • Adept at dealing with contract and provider relationships.
  • Customer service focused: Able to respond promptly to customer needs; solicit customer feedback to improve service; respond to requests for service and assistance; and meet commitments.
  • Knowledge of Microsoft applications (Word, Excel, PowerPoint, Outlook).
  • Proficient in call center technologies/customer service management systems.
  • Experience in a call center environment.
  • Experience with individuals diagnosed Serious Mental illness is preferred.
  • Bi-lingual preferred but not required.
