Responsibilities The PPS / Referral Coordinator facilitates admissions to the Rehabilitation Unit. The position involves acting as liaison between the physician, staff and rehabilitation unit to facilitate the admissions process for internal and external referrals. PPS / Referral Coordinator completes FIM scores day four and on discharge. Responsible for staff education and FIM recertification. Transmits data to CMS for reimbursement, maintains transmission log and coordinates this with coders and patient accounting who submits the UB04. Remains current with CMS rules and regulations. Qualifications Education and Work Experience 1. Current and valid Michigan Registered Nurse licensed, Physical Therapist, Occupational Therapist, or Speech Language Pathologist licensure. 2. BCLS certification. 3. A minimum of two years clinical experience in the rehabilitation of chronic or acute patient certification preferred.
