Overview

NaphCare is hiring a Case Manager to join our Jail Based Treatment Program team of medical professionals in the Mesa County Detention Center located in Grand Junction, Colorado.

The Jail-Based Treatment Program (JBTP) provides specialized behavioral health services for patients awaiting transfer to a state hospital for further psychiatric treatment. The unit is a therapeutic environment, providing intensive day treatment services. By providing patient-centered, evidenced-based, and holistic care, the JBTP seeks to eliminate the need for these patients to transfer to the state hospital or, at a minimum, to decrease their length of stay once hospitalized. The ultimate aim is to assist defendants in moving forward with their legal case and with their lives in a healthier place.



Responsibilities

Case Manager duties and responsibilities may include and are not limited to:

    Serve as a contributing member of the multidisciplinary treatment team
  • Completes initial discharge/case management plan and updates, and discharge packet
  • Facilitates information exchange with State Hospital and community service providers
  • Identifies and seeks receipt of patients' collateral records
  • Identifies and documents contacts with patients' collateral contacts
  • Collaborates with facility (e.g., off-program medical and mental health staff; deputies; discharge planner) and State Hospital staff to coordinate patient transfers
  • Conducts at least a weekly psychoeducational group for JBTP patients
  • Meet individually with the JBTP patients regularly to use Motivational Interviewing to assist patients with engagement in treatment; provide psychoeducation and self-paced journaling; and assist patients with the identification of a support network, instructions for future care and follow-up, and concerns, complaints, and requests regarding care upon discharge
  • Assists with administering patient outcomes assessments (e.g., questionnaires)
  • Coordinate care/build relationships with outside agencies as necessary for discharge planning
  • Relay critical information to the next level of management that may have a negative impact on client and/or vendor relations or patient care
  • Participate in educational and training activities related to mental health care for patients, security personnel, and healthcare staff


Qualifications

Qualifications for a Case Manager:

  • Bachelor's Degree in a human service field
  • Correctional healthcare experience preferred but not required
  • Valid CPR card
  • Knowledge in the development of discharge plans and coordination with community service providers
  • Effective communications skills are required for building patient rapport and relaying information and treatment plan with other medical and mental health care professionals
  • Strong organizational skills are needed to keep track of patient treatment plans and progress notes
  • Must have the ability to maintain confidentiality of all proprietary and/or confidential information and integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements
  • Must pass the facility's criminal background check and drug screening

Physical Capabilities:

Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
