Responsibilities
Job SummaryEnsure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes for HonorHealth patients in various healthcare settings including home, hospital, or infusion center. Performs the duties of a registered pharmacist as defined by the Arizona State Board of Pharmacy, which includes appropriate selection, dosing, administration, sterile compounding, clinical assessment, and dispensing of medication orders. Provides support for clinical programs, staff training, quality improvement, and patient education.
Essential Functions
- Pharmacy Operations and Order Verification Interpret and evaluate home infusion medication orders to ensure appropriate therapy, dosing, frequency, route, and duration of therapy in accordance with clinical guidelines, State and Federal regulations, and other regulatory organizations.
Verify orders for parenteral nutrition, anti-infectives, inotropes, chemotherapy, pain management, immunoglobulin (IVIG/SCIG), and other specialty infusion therapies.Reads, extracts, and interprets information in patient charts accurately.Enters/verifies medication orders in the computer processing system accurately and efficiently.Clarify incomplete or unclear orders promptly and communicate with prescribers for resolution.Coordinate with intake and nursing teams to ensure medication therapy aligns with patient care plans.Maintain patient-specific infusion records in EMR and pharmacy systems.Act as a liaison with providers, case managers, and nursing staff to resolve clinical issues and ensure seamless care coordination.
- Compounding and Dispensing
Compound and supervise the compounding of sterile IV medications (e.g., antibiotics, TPNs, chemotherapy, pain management) in accordance with USP <797>, USP <800>, and company protocols.Ensure accurate aseptic technique and compliance with cleanroom standards and PPE use.Oversee technician staff preparing infusion admixtures, providing instruction and final product verification.Coordinate delivery schedules and packaging to meet clinical timelines and maintain product integrity.Manage medication and supply inventory to support efficient pharmacy operations and patient care.Assures the quality of products dispensed meets established departmental and regulatory guidelines.Accurately re-establish priorities based on compounding and dispensing workload with consideration of patient needs.Programs patient specific pumps for in-home use.
- Clinical Monitoring and Therapeutic Support
Monitor clinical efficacy, side effects, and laboratory results to guide therapy adjustments.Evaluate and recommend dose adjustments based on renal/hepatic function, lab data, or patient tolerance.Provide TPN assessment and adjustment based on laboratory trends and nutritional goals.Participate in interdisciplinary care discussions and collaborate with physicians and nurses to optimize patient outcomes.Provide patient and caregiver education on proper storage, administration, side effects, and infusion device usage.Manage clinical follow-ups and coordinate with nursing on changes to therapy or care plans.
- Compliance, Quality, and Documentation
Ensure compliance with Board of Pharmacy, Joint Commission, USP, and other applicable standards.Maintain accurate and timely documentation of interventions, dispensing records, compounding logs, and clinical assessments.Support internal audits, accreditation surveys, and quality assurance activities.Participate in the maintenance and review of policies and procedures related to infusion pharmacy practices.Identify and report adverse drug events (ADEs), medication errors, and near misses per policy.
- Assist in onboarding and training of new pharmacists and technicians on home infusion procedures.
Provide ongoing education to internal staff and external providers about drug therapy and infusion best practices.Precept pharmacy students or residents when applicable.Stay current on advancements in home infusion therapy and related regulatory changes.Participate in departmental and organizational quality improvement initiatives.Pursue continuing education and certifications relevant to infusion pharmacy and sterile compounding.Engage in initiatives that promote patient safety, cost-effective care, and best practices in pharmacotherapy.
Education Bachelor's Degree BS Pharmacy Degree from an ACPE-accredited School of Pharmacy -

Experience1 to 3 Years experience as a hospital Pharmacist, home infusion Pharmacist, or Pharmacy residency (PGY1) - Preferred
