Description

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being, and are looking for a Preventive Clinical Program Manager in our Community Based Services Program.

Position Summary:

The Preventive Clinical Program Manager provides direct clinical services to youth and families in the mandated preventive program. Clinical services include but are not limited to individual, group and family therapy including assessment, evaluation and treatment. The Preventive Clinical Program Manager also supervises the Preventive Program Caseworkers.

Schedule: 35 hours per week, Monday - Friday with rotating on-call responsibilities

Potential for hybrid schedule after successful completion of probationary period

Essential Functions and Responsibilities:



  1. Provides individual, group and family therapy to youth and families.

    1. Holds a caseload of 30 youth/families.
    2. Conducts face to face therapy sessions with frequency depending on acuity level.


  2. Conducts safety and clinical assessments and evaluations for families in the preventive program.

    1. Uses formalized assessment templates including but not limited to:

      1. CANS
      2. Biopsychosocial
      3. Child and Adolescent Trauma Screen - Caregiver (CATS-C) / 3-6 years
      4. Child and Adolescent Trauma Screen (CATS) / 7-17 years
      5. Child and Adolescent Trauma Screen - Caregiver (CATS-C) / 7-17 years
      6. CRAFFT Screening Tool for Adolescent Substance Abuse
      7. Mood and Feelings Questionnaire / Child Self-Report
      8. Mood and Feelings Questionnaire / Parent Report on Child
      9. Patient Health Questionnaire (PHQ-9)
      10. Screen for Child Anxiety Related Disorders (SCARED) / Child Version
      11. Screen for Child Anxiety Related Disorders (SCARED) / Parent Version
      12. Columbia-Suicide Severity Rating Scale (C-SSRS)


    2. Conducts safety and risk assessment upon each visit to the family.


  3. Provides, and supports Preventive Program Caseworker in providing, strengths-based service planning, youth-guided care and psychoeducation to youth and families.
  4. Monitors performance indicators (noted below) to be sure the program is successful.

    1. Provides data to administrative assistant to track and report


  5. Participates in monthly case record review meetings with Dutchess County LDSS
  6. Provides supervision to Caseworker staff.

    1. Meets with caseworkers weekly to review case progress and provide quality care


  7. Fills in gaps of care in the event a staff member calls out.

    1. Covers or reschedules family visits should a caseworker be unavailable


  8. Provides and supports Casework staff in providing solution-focused interventions using evidence based practices.

    1. After the initial case assessment the Lead Clinician will help caseworkers to identify appropriate interventions with the youth/families.
    2. Evidence practices include but are not limited to: motivational interviewing, and Functional Family Therapy.
    3. SafeCare intervention is an in-home behavioral parenting program that promotes positive parent-child interactions, informed caregiver response to childhood illness and injury, and a safe home environment.


  9. Service provision to include supporting youth and families with community integration, specific mental/behavioral health interventions, concrete needs and social and interpersonal skills.
  10. Completes case notes within 24-72 hours of service provision as needed.
  11. Collaborates with families, schools, and other service providers to successfully provide services.
  12. Participates in weekly supervision.
  13. Ongoing training(s) as necessary.
  14. All other duties as assigned.

Qualifications

MINIMUM Qualifications:



  • MSW or MHC required
  • At least 2 years of supervisory experience
  • At least 1 year of experience working with youth and families.
  • Valid NYS Driver License in good condition as determined by the Agency
  • Must complete and pass a pre-employment physical, PPD and drug screen and an annual physical and PPD.


PREFERRED Qualifications:



  • LMSW, LCSW, LMHC preferred
  • Experience working with youth and families touched by the child welfare system
  • Experience working with temporary assistance, SNAP, Food stamps etc.
  • Trained in FFT or Safecare models.
