Service Coordinator

The working hours for this part-time position will be 9am-4pm, Monday, Wednesday and Friday

Starting salary is $25 per hour

Summary/Primary Purpose: To build on strengths, develop skills to manage situational change, access appropriate community/state/federal resources for the older adult consumer, and serve as liaison and advocate.

Essential Duties and Responsibilities:



  • Complete intake assessment, current evaluation of community resources and income verification of new program participants.
  • Establish, update, and maintain file documentation and service coordinator database.
  • Provide crisis intervention and community referral services as needed.
  • Serve as liaison and advocate between agency providers and residents.
  • Provide leadership in monthly Multidisciplinary Team meetings
  • Gather and report statistical data for The Salvation Army and other funders.
  • Conduct home visits as needed for evaluation and follow-up.
  • Update the Emergency Form on all DBM residents on a bi-annual basis.
  • Update and maintain the Resource binder on an annual basis.
  • Coordinate the Loan Closet (medical assistive devices) as equipment is donated or checked out.
  • Locate area farmers to host Famers Market at DBM and coordinate the distribution of summer Farmers Market vouchers.
  • Coordinate application for government commodities program to eligible participants and coordinate monthly distribution.
  • Serve as "Task Supervisor" for practicum students.
  • Assist with DBM Bedbug preparations as needed.
  • Participate in performance and quality improvement program.
  • Prepare gift bag and deliver birthday gifts to residents.
  • Assist with Social and Recreational department activities.
  • Assist with various holiday programs as needed.
  • Willing to utilize trauma informed practices.
  • Assist residents how to access information and services online.
  • Driving is an essential function of the position.
  • All other duties as assigned


Supervisory Responsibilities: N/A

Education and/or Experience: Bachelor's degree (B. A.) in Social Work or Human Services from four-year college or university and minimum 3 years related experience with older adults and community resources.

  • Have excellent communication and problem solving skills.
  • Be organized, able to work independently and in a team environment.
  • Understand the delivery of social services and eligible benefits.
  • Ability to work well with clients of diverse cultural and socioeconomic groups; respects confidentiality; able to work within ethical and legal parameters of social services.
  • Willing to support the philosophy, goal, mission and objectives of The Salvation Army.
  • Complete licensing for Service Point and Pathway of Hope.
  • Administrative skills to include utilization of standard office equipment, data entry, 10-key, attention to detail and filing.
  • Intermediate skills in Microsoft software to include Word, Excel, PowerPoint and Access. As well as the ability to access payroll, timekeeping and personal data via a web-based system.

Physical requirements include: good speaking, hearing and vision ability and excellent manual dexterity. Ability to stand, bend and reach for extended periods of time, dexterity of hands and fingers to operate office equipment. May need to climb, balance. Kneel, crouch or crawl. This position will required the ability to lift and/or move more than 50 pounds occasionally.

Working Conditions: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job - an office environment and while performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
