Overview Physical Therapist PACE (Program of All-Inclusive Care for the Elderly) Monday-Friday Full Time Benefits $10,000 Sign On Bonus, if external The PACE Physical Therapist functions as an integral part of the PACE team and has a responsibility for evaluating and treating patients requiring services associated with physical therapy. The Physical Therapist is expected to communicate with the PACE Interdisciplinary Team regarding the patient's treatment plan and progress, and deliver services consistent with policies and procedures governing the delivery of patient care by the PACE program. Responsibilities * Perform examination and evaluation to determine diagnosis and prognosis of patient/client and determine appropriate intervention to gain expected outcomes based on patient/client goals. * Utilization of appropriate Physical Therapy Interventions to produce expected outcomes. * Utilization of appropriate documentation to communicate progress and status to PACE interdisciplinary team members, including physician, nurses, social workers, occupational therapist, recreation, dietary, home care, etc., in order to coordinate and manage care of PACE participants living at home, a nursing/skilled nursing facility. Follows through with recommendations of IDT team. Works with various vendors to meet medical equipment needs of participants. * Maintains professional code of ethics and state licensure laws. * Supervision of Physical Therapist Assistant in accordance to state licensure laws. * Keep work area and equipment clean and orderly by maintaining and practicing hospital and department policies and procedures regarding safety and infection control. * Performs as member of PACE team through joint interventions, communication, quality assurance programs and meetings. Qualifications Education Graduate of an accredited physical therapy program License(s)/Certification(s) Valid driver's license when driving any vehicle for work-related reasons.
