This is an inpatient-based Case Management/Care Manager training program which includes both virtual classroom and precepted hours. The program is 8 weeks in length. Successful candidates will transition into full time Inpatient Care Manager positions across the Scripps system.

Role Description: Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet a patient's and family's comprehensive health needs through communication and available resources.

Using objective data, evidence-based screening tools and care team referrals, the Care Manager screens all patients in their assigned caseload to identify those patients who:



  • may have discharge needs
  • may be at risk for an unplanned readmission within 30 days of acute care discharge
  • have a high risk of mortality
  • are medically complex and may have a prolonged LOS
  • have payer issues


The Care Manager will coordinate with the health care team and the patient/family to address payer issues that impact care, develop a transition plan of care that includes a safe discharge plan, and plan for follow-up with care providers post discharge. The Care Manager ensures that the patient and family agree with and understands this transition plan, and that the plan is implemented appropriately.

The following locations are hiring for this program, and candidates will be able to select their location preference during the application process:



  • Scripps Memorial Hospital La Jolla
  • Scripps Mercy Hospital San Diego (Hillcrest)


Program Dates: 8/11/2025 - 10/4/2025
Application Period: 4/20/2025 - 5/3/2025
Interview Period: 5/12/2025 - 6/6/2025

Interviews:



  • Qualified candidates will receive an invitation to record a HireVue On-Demand Video Interview. The link to complete the video interview expires 72 hours from the time the invitation is emailed to the candidate.
  • Hiring leaders will review these videos and invite selected candidates to schedule an interview during the interview period.


Selected Candidates:



  • New hires/rehires must be available to start no later than 7/28/2025
  • Must be available to complete the entire program - class attendance is mandatory
  • NOTE: There is no paid time off (PTO) allowed during the program.

Qualifications

Basic License/Certification Requirements:



  • Current CA RN License
  • Current BLS for Health Care Provider from the American Heart Association


Minimum Experience/Skill Requirements:



  • A minimum of 2 years recent clinical experience as a Registered Nurse
  • Excellent communication skills
  • Must be able to speak, read, and follow instructions in English
  • Excellent interpersonal skills


Preferred Qualifications:



  • BSN preferred
  • Case Management and/or home care experience
  • Case Management certification highly preferred
