Summary

The Hospice Clinical Care Manager provides innovative thinking and gains strategic buy-in from key stakeholders to support the delivery of effective patient care through the management of company clinical resources, processes, and supplies. This position will leverage internal and external systems to elevate visibility on reduction of cost through improved clinical therapy adherence, waste mitigation and education. The Hospice Clinical Care Manager will understand hospice regulatory requirements, clinical and symptom management needs of Hospice patients, and supportive care needs of their families.

Must be able to travel nationwide up to 60% weekly.

3 years Hospice Case Manager or Hospice Supervisory experience required.



Responsibilities
  • Provides insights into trends in the delivery of care and clinical operation efficiencies.
  • Develops with Regional Vice President of Hospice and Hospice Quality team initiatives to adjust clinical operations, expenses and staffing to address needs in quality improvement and fiscal metrics.
  • Works directly with hospice agencies leaders and care teams providing tools and education that drives improved operational outcomes.
  • Engages regularly, both on-site and remotely, with Hospice providers to address opportunities for improvement, through participation in activities such as Morning Stand Up calls, IDG meetings, patient visits, etc.
  • Escalates and communicates issues through appropriate channels.
  • Ensures thorough execution and documentation of key activities and initiatives.
  • Supports change management, ensuring clinical care teams are well prepared and versed on hospice standards of practices.
  • Develops training courses in collaboration with the education department on clinical topics that directly impact clinical outcomes and operational efficiencies.
  • Engages with internal LHC Group teams such as the Compliance and Quality department team members to ensure awareness of, and compliance with, regulatory requirements and updates, and to serve as a resource for Hospice agency staff.
  • Participates regularly in professional activities and user forums to keep abreast of new developments in hospice clinical practice and standards.
  • Always observes patient confidentiality.
  • Performs other tasks as assigned.


Education and Experience
  • Associate or bachelor's degree in nursing.
  • Must have at least three years of hospice experience, in a RN Case Manager role or hospice leadership role.
  • Excellent verbal and written communication skills with all members of the healthcare team.
  • Excellent organizational skills and ability to complete competing priorities.
  • Must have thorough understanding of hospice coverage guidelines.
  • Must have strong clinical and symptom management skills, specific to hospice and palliative care patients.
  • Excellent presentation, negotiation and relationship-building skills required.
  • Must have strong computer skills.
  • Completes on-going training on-the-job, through courses, self-study, certifications and/or advanced degrees to maintain and enhance technical and business capabilities.
  • Highly driven and self-motivated to exceed expectations.
  • Ability to work independently and in a team-based environment.
  • Must have current registered nursing license in state of practice.
