PD Social Worker - Toms River, NJ

Job Overview:

The Social Worker provides direct clinical social work services to patients and their families in an interdisciplinary team approach. Responsibilities include but are not limited to: Completing initial social assessments on assigned patients, providing social work counseling with individuals, family or groups as needed, providing social work consultations as needed, providing patient centered advocacy, collaborating with interdisciplinary teams, managing complex discharge plans, building and maintaining relationships with internal/external resources, making referrals to community agencies and entitlement programs and continuing ongoing education. Ensures continuity of care for patients with complex post-acute needs and maximizes the patients' and family's abilities to meet the continuing needs within their social and financial framework. Understands all stages of growth and development to assess each patient s age-specific needs to provide individual treatment and care.

Qualifications:

Required:

• Master's Degree in Social Work
• LSW/LCSW

Scheduling Requirements:

• Day Shift
• Per Diem (schedule will vary based on the needs of the department

Essential Functions:

• Complete initial psycho-social assessments on assigned patients
• Provide psychotherapeutic counseling with individuals, family or groups as needed
• Clinical consultations as needed
• Provide patient centered advocacy, collaborating with interdisciplinary team, managing complex discharge plans
• Making referrals to community agencies and entitlement programs
• Ensure continuity of care for patients with complex post-acute needs and maximizes the patients' and families abilities to meet the continuing needs within their social and financial framework
• Preforms other duties as assigned
