How Is This Role Important to Our Work?

The position supports the Medicare Medication Therapy Management (MTM) program that aims to optimizing medication regimens for seniors with complex medication needs. This includes patients at risk for missing doses of their medications, those most likely to inappropriately use their medications and those at high risk for adverse events from their medications due to multiple conditions, seeing several doctors, or going to multiple pharmacies. This role works to call seniors and schedule telephone based medication reviews with our Medication Therapy Management pharmacists. This position will receive targeted member lists, perform outreach to those members with the intent of scheduling an appointment with the MTM pharmacist, and will use a software platform to document all required data elements to assist in the documentation of the care provided by the MTM pharmacist. This role requires approximately 75% of the time to be spent on the phone calling members.

A Day in the Life:
  • Review clinical data files to perform outreach to targeted members.
  • Participate in training sessions, specific to software platform.
  • Enter data elements into software platform.
  • Research updated contact information for members.
  • Check completed work for errors.
  • Communicate with partner teams to coordinate data entry audits and required regulatory reporting.
  • Support MTM pharmacist in completing documentation of MTM services and delivery of patient and provider reports.
Nice to Have:
  • Experience in a health care related setting.
  • Experience and knowledge working with MTM program(s) in a clinic/retail setting or with a PBM.
  • Excellent verbal and written communication skills.
  • Strong problem solving skills.
  • Learns quickly and adapts to change.
  • Acquires and maintains high accuracy and attention to detail.
Required Skills and Experiences:
  • 1+ years of related experience. All relevant experience including work, education, transferable skills, and military experience will be considered.
  • 1 year experience working with members/clients/patients on the phone.
  • Proficient experience using computer applications.
  • Outstanding customer service skills.
  • Organized and efficient in completing tasks and meeting timelines.
  • Communicates well with multidisciplinary team.
  • Contributes to process improvements by suggestions for continuous improvement of departmental procedures.
  • Completes special projects as assigned.

Compensation and Benefits:

Pay Range: $21.00 - $23.86 - $28.63 Hourly

Pay is based on several factors which vary based on position, including skills, ability, and knowledge the selected individual is bringing to the specific job.
