The shift for this vacancy is 4pm - 12am, Monday - Friday; located at the new Crisis and Recovery Center in West St. Paul

Under supervision and within state and locally developed programs and policies, identifies individual, family, and community problems and selectively uses individual or family casework, group methods, or community organizations to work toward the rehabilitation, prevention, or control of these problems.

Minimum Qualifications

NEED TO HAVE QUALIFICATIONS:
- Bachelor's degree in Social Work, Psychology, Sociology, or related Human Services field
AND
- One year full-time professional experience in a social services agency OR Two years full-time paraprofessional experience in a social services agency.
AND
NICE TO HAVE QUALIFICATIONS:
- Master's degree in Social Work, Psychology, Sociology, or related Human Services field
- Previous experience in completing initial screening and assessment in one or more of the Adult Service Program areas: Adult Protection, Chemical Health, Mental Health, Housing, and Pre-Petition Screening.

Duties & Responsibilities

- Determine with the client, and in collaboration with agencies, eligibility, need, and level of service for identified clients.
- Provide, arrange, and coordinate direct services and advocacy to clients and families.
- Facilitate, integrate, and mobilize agency and community resources on behalf of the client.
- Complete closure of case management activity with client based on resolution of established goals and/or successful integration to community resources. Terminate activity with client when client requests closure or when involuntary closure is ordered.
- May conduct initial mental health screening of adults, children, and adolescents in crisis via phone to determine client needs.
- Collaborate and consult with supervisory mental health professionals, colleagues, case managers, and family members with the appropriate authorization.
- During non-business hours, act as county response representative supporting efforts of adult protection, child protection and crisis response with the support of supervisors and colleagues.
- Research, maintain, and apply knowledge of relevant policies, procedures, and statutes. Provide training and education to others.
- Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.
- Demonstrates a commitment to support cultural diversity County wide, actively supports the County's inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities.
- Perform other social services duties as assigned.

Knowledge, Skills & Abilities and Work Environment

* Knowledge of child and human development, family dynamics and systems, including crisis management and cultural competency. * Knowledge of community resources and funding sources. * Knowledge of medical diagnoses, medications, and basic medical terminology, as well as medical insurance processes and procedures. * Knowledge of statutes, rules, and policies pertaining to social work and core job functions (e.g. Vulnerable Adult Act, Rule 79 Adult Mental Health Case Management) * Interviewing, negotiation, and listening skills. * Ability to communicate effectively both orally and in writing. * Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English.
WORK ENVIRONMENT: Work is performed in a County office building answering calls and in person interactions. Lifting requirement of up to 20 pounds on an occasional basis. Equipment used may include, but is not limited to, computer, printer, telephone, cell phone, and copy and fax machines.
