DESCRIPTION

Pharmacy Home Infusion Services (PHIS) prepares sterile IV admixtures for home infusion patients and ambulatory infusion suite (AIS). PHIS uses specialized computer programs (CareTend, Abacus, PrecisePK, EPIC) and equipment (EM2400, Repeater pump) that requires specialized training for the Pharmacists. Under general supervision, responsible for provision and documentation of medications and medication-related services to patients, according to standards established by the Department of Pharmacy.

The Pharmacist is responsible for providing clinical pharmacy services to home and AIS patients (e.g. Hemophilia, Antibiotics, TPN, Inotropes, Remicade, Entyvio, Tysbari, Enzyme replacement, IVIG, and other biologics). Possess knowledge of IV catheters and devices for administration of IV medications. The Pharmacist is expected to have basic understanding of insurance benefits, prior authorization, billing of Medical and Pharmacy claims. Serve as preceptor to pharmacy residents and students as assigned. Participate in Medical Center and Departmental committees and promote institution wide medication use policy.

MINIMUM QUALIFICATIONS
  • Doctorate of Pharmacy Degree or equivalent clinical experience.

  • Registered Pharmacist - issued by the state of California (RPH).

  • Pharmacy residency or equivalent clinical experience.

PREFERRED QUALIFICATIONS
  • Experience in Home Infusion Pharmacy practice.
  • Pharmacokinetic monitoring/dosing and experience with prior authorizations.
  • Intravenous medications and sterile compounding.
SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.

Pay Transparency Act

Annual Full Pay Range: $179,631 - $202,244 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $86.03 - $96.86

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
