Hourly Wage: $26-32 an hour, depending on experience

Position: Clinical Liaison LPN - Lifespark Complete Senior Living

Coverage area will be in the west metro: Delano, Carver & one of our AL buildings; Yorkshire of Edina Senior Living. Plus mileage reimbursement of .70 cents per mile. This is a hybrid role. You must be able to travel to 1 site/location at least once a day (this position is a combination of remote/work in-home environment and travel to our community, TCU, LTC, Assisted and Independent Living partnered sites).

Hours: Full-Time / Monday through Friday, 8am-5pm. Full-time, 1.0 FTE

The Clinical Liaison is primarily responsible for assisting with the implementation and execution of the Lifespark Health primary care services provided to our clients. In this position you will be a part of an energetic, positive team that assists our Nurse Practitioners and Physicians in delivering exceptional primary care for our members in the community, TCU, LTC, Assisted and Independent Living partnered sites. This position requires excellent communication and customer service skills, a team player approach and the ability to perform in a fast-paced environment. The ideal candidate would have experience with EMR's such as Athena and PCC, computer software skills, highly organized and positive. Clinical experience in a nursing home/TCU/Assisted Living, Home Care or Clinic setting is required. This position will offer direct patient care along with communication with providers/clinical team, facility staff, and chart documentation.

Position Functions & Responsibilities:

• Perform excellent customer service to clients, customers, partners and colleagues daily
• Documentation of visits and coordination details in EMR
• Direct communication with MD/NP/LM/Managers for urgent client needs.
• Assist with medication reconciliation, vital sign entry, skilled interventions as ordered.
• Assist members with urgent needs within the scope of practice of LPN.
• Participate in team meetings.
• Collaborate with clinical team and assist with exams/procedures as needed.
• Provide coverage when needed for other clinic staff
• Timely response to inquiries by telephone, EMR, Microsoft TEAMS, email, or fax
• Provide team coverage to other departments as needed
• Completes all documentation in a timely manner and responsible for client record content
• Maintains a strict standard of confidentiality to ensure our member's protected health information (PHI) is secure

Qualifications:

Education-

• Licensed Practical Nurse with a minimum of 1 year of experience; community-based services preferred.
• College degree in healthcare or business field preferred.
• Experience in customer services/direct client contact field preferred
• Experience using technology including Microsoft Office, Smart Phone, Outlook, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing
