Job Description:Under the supervision of the Care Coordination and Resource Services Supervisor, the Patient Navigator is responsible for helping patients maximize the health status of older adults. The Patient Navigator assesses the patient's needs that may include education or community resource information. Advocates for patients and serves as a guide to obtain support or services needed in John Muir Health and within the community. Offers guidance and support, empowering patients to make informed treatment decisions. Demonstrates consideration of age-specific issues relating to meeting the needs of older adult patients. Assists in patient care services needed to increase patient satisfaction, reduce complaints and grievances regarding billing and insurance matters. Collaborates with all John Muir Health departments to facilitate provision of high quality, cost effective medical care. Assures patient safety, privacy, and confidentiality.

Pay Range: $31.60 - $42.67 HourlyOffer amounts are based on
demonstrated/relevant experience and/or licensure.
Pay will be adjusted to the local market if hired outside of the Bay Area.
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
