Wage Range: $28-$34 an hour, depending on experience + Pay Per Visit (PPV), rates as follow: Evaluation: $85 & Routine Visit: $70

Location: Twin Cities Greater Metro. This position requires that you drive covering the entire metro area; northwest area: St. Michael, Albertville, Rogers, Maple Grove, to the southwest area: Chaska, Eden Prairie, Edina, Savage, to the southeast area: Hastings, South St. Paul, Eagan, Cottage Grove, Afton, to the northeast area: Forest Lake, Woodbury, White Bear Lake, Hugo, Lino Lake and in-between. Plus mileage reimbursement of .70 cents per mile.

Schedule: Casual/Part-Time/PPV ~ average of 4-10 home visits a week

The Dietitian is primarily responsible for providing service in accordance with a plan of treatment and care plan for the Home Health Agency.

Position Functions & Responsibilities:

• Observes and evaluates treatment effect and client's overt physical/emotional response to treatment goals. Updates treatment plan based on evaluation and as new orders are received
• Assesses the rehabilitative status of the client and develops appropriate treatment plan for each client; updates and modifies treatment plan on an ongoing basis
• Lead by example in establishing rapport with clients
• Ensure positive & professional communication on team
• Plans goals and care communicates with IDT
• Observes, records and reports information on the client's condition to the attending physician and in the client's medical record
• Documents client treatment and response to treatment accurately, using established guidelines
• Honors client confidentiality, rights, privacy, and reporting maltreatment expectations
• Follow all Emergency and Response Procedures
• Identifies home safety issues and notifies RN Case Manager or Clinical Manager of concerns or issues
• Demonstrates proper procedures: handling, lifting, transfers
• Injury - Now What? Follows protocols
• Overview of Process, Types of Reports and Forms
• Follow-up Required - Escalating Event
• Other duties as assigned

Qualifications:

• Experience working with people in the second half of life.
• Current and active Dietitian license in the state of Minnesota.
• Experience in geriatrics a plus!
• Background in skilled nursing, hospitals or home health preferred.
