Overview

Provides nursing care for patients and assisting physicians. Clinical and administrative duties include: triage patients vital signs, height/weight, medical/social history, medication reconciliation, stock/prepare exam rooms; retrieve imaging and laboratory results; process work task/message ques in the electronic health records [Allscripts], and the patient portal; Follow My Health; and ability to work in our clinical and administrative areas with the Department of Medicine



Responsibilities

Triage incoming patients by obtaining vital signs, reconcile current medication list, reconcile
allergies, and determine chief complaint.

Administer injections of medications or immunizations in accordance with approved nursing
protocol or standing orders and document appropriately in the patients' electronic medical
record. Observe patient for any adverse reactions.

Assist physicians with examinations, minor surgeries, diagnostic procedures, and treatments.
Implement standing orders when applicable. Provide basic care as ordered.

Appropriately document all nursing interventions in the electronic medical record.
Review chart for completeness, test results, task completion, and billing
Provide patient teaching/education with medications.

Review and work/complete all "tasks" assigned to you and your individual team (as well as
covering for other nurses who may be absent). This will include responding to patient
messages, prescription refills, lab results, referrals, home health agencies, medication prior
authorizations, patient forms and follow up from physicians.

Schedule specialist appointments and obtain referral from insurance companies as needed.
Obtain prior authorization for medications.

Obtain lab/CT/MRI/Biopsy results from outside sources, as needed and scan into the patient's
electronic medical record. This includes reviewing upcoming appointments and obtaining
prior to the visit.

Notify providers with critical labs

Call test/lab results to patients and answer questions regarding those results and physician
instructions.

Provides Tuberculin purified protein derivative (PPD) injections as needed.

Respond to questions from patients, family members, faculty/residents, other health care
providers in a timely and professional manner. Patient phone calls should be answered or
documented the same day they are received. All patient conversation must be documented in
the chart, as well as attempts to reach patient.

Patient education, discharge medication reconciliation, drug assistance programs nutrition and
barriers to care.

Distribute and track Home Health orders and key in Home Health charges for billing.

Participate in staff meeting and training sessions/education lectures.

Other duties as assigned within the office and Department of Medicine



Qualifications

Completion of a program of instruction for licensure as a Practical Nurse required. Completion of BLS Class and re-certify every 2 years. Must be a licensed LPN in Virginia. Previous clinical experience preferred. Montreal Cognitive Assessment (MoCA) certified desirable.
