Licensed Practical Nurse II
Overview Provides nursing care for patients and assisting physicians. Clinical and administrative duties include: triage patients vital signs, height/weight, medical/social history, medication reconciliation, stock/prepare exam rooms; retrieve imaging and laboratory results; process work task/message ques in the electronic health records [Allscripts], and the patient portal; Follow My Health; and ability to work in our clinical and administrative areas with the Department of Medicine Responsibilities Triage incoming patients by obtaining vital signs, reconcile current medication list, reconcile Administer injections of medications or immunizations in accordance with approved nursing Assist physicians with examinations, minor surgeries, diagnostic procedures, and treatments. Appropriately document all nursing interventions in the electronic medical record. Review and work/complete all "tasks" assigned to you and your individual team (as well as Schedule specialist appointments and obtain referral from insurance companies as needed. Obtain lab/CT/MRI/Biopsy results from outside sources, as needed and scan into the patient's Notify providers with critical labs Call test/lab results to patients and answer questions regarding those results and physician Provides Tuberculin purified protein derivative (PPD) injections as needed. Respond to questions from patients, family members, faculty/residents, other health care Patient education, discharge medication reconciliation, drug assistance programs nutrition and Distribute and track Home Health orders and key in Home Health charges for billing. Participate in staff meeting and training sessions/education lectures. Other duties as assigned within the office and Department of Medicine Qualifications Completion of a program of instruction for licensure as a Practical Nurse required. Completion of BLS Class and re-certify every 2 years. Must be a licensed LPN in Virginia. Previous clinical experience preferred. Montreal Cognitive Assessment (MoCA) certified desirable.
