Patient Navigator - MATER

Care Navigators provide care coordination to future, current, and past individuals seeking for substance use disorder treatment. Navigators connect individuals to substance use disorder treatment including comprehensive behavioral and physical health services, and facilitate engagement and reengagement in these services. Further, navigators coordinate appointments and receipt of additional services, such as but not limited to eligilbility and insurance coverage, food benefits, housing, and other case management services. This job requires flexibility of time and location to engage individuals in hospitals, assessment centers, substance use disorder treatment facilities, and other community locations.

Job Description

ESSENTIAL FUNCTIONS:

  • Initiates the intake process for individuals seeking treatment at MATER, including but not limited to the completion of the intake paperwork and assessments. Coordinates intake for those seeking care outside of MATER.
  • Assesses physical, behavioral, social, health, and case management needs and barriers through semi-structured interviews and provides referrals and assistance making appointments with appropriate organizations.
  • Per program procedures, assesses individual engagement in substance use disorder treatment and related care, using evidence-based practices (e.g., motivational interviewing, and related techniques to reengage those who have disengaged.
  • Keeps current with local, state, and federal regulations regarding confidentiality of information pertaining to both medical and drug and alcohol treatment records.
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

EDUCATIONAL/TRAINING REQUIREMENTS:

Bachelor's degree in psychology, social work, health services, or related field from an accredited college/university. Master's degree preferred.

CERTIFICATES, LICENSES, AND REGISTRATION:

Driver's License required.

EXPERIENCE REQUIREMENTS:

Minimum 2 years' experience in drug & alcohol treatment, mental health, health insurance claims, claims management, or patient navigation preferred.
