Description

Lutheran Social Services is currently seeking a Human Services Professional to specialize in the our Family Centered Treatment (FCT) team! FCT is a home-based method of treatment that focuses on 4 phases; Joining and Assessment, Restructuring, Valuing Change, and Generalization. Providers in the program see the entire family unit as defined by the family as the client and conduct several multi-hour sessions are conduct in the home weekly.

This position performs work involving assessment, diagnosis, and treatment of individuals and their family with mental health, substance use, or co-occurring disorders within the evidenced-based model of FCT.

The Human Services Professional - Specialized provides treatment to families under the oversight of the assigned Licensed Therapist - Specialized or Therapist In-Training - Specialized. The services provided include Individual and Family Psychoeducation, Individual Skill Development and Enhancement. The services are provided within the employee's scope of practice.

Additional compensation incentives when FCT training is complete and productivity expectations are met.

ADDITIONAL JOB TITLES THAT ARE USED IN THE FIELD (not all inclusive):



  • Social Worker (This title is reserved for those with a specific degree or credential in social work.)

Examples of services Expected may include (These are examples, not all inclusive, and all services may not be provided by an individual employee.):



  • Individual Skill Development and Enhancement (e.g. daily living skills - cleaning, hygiene, mentoring of youth, teaching of parenting skills, community engagement)
  • Individual and/or Family Psychoeducation (e.g. mental health, substance use, parenting)
  • Medication Monitoring - Non-Prescriber (e.g. delivery medications to improve compliance, review and teach clients about the medications/potential side effects)
  • Employment Related Skill Training - address the person's illness or symptom-related problems in finding, securing, and keeping a job. Services may include but are not limited to: employment and education assessments; assistance in accessing or participating in educational and employment- related services; education about appropriate job-related behaviors; assistance with job preparation activities such as personal hygiene, clothing, and transportation; on-site employment evaluation and feedback sessions to identify and manage work related symptoms; assistance with work-related crises; and individual therapeutic support.
  • Participation on recovery teams. (provide feedback around service delivery and progress)
  • Crisis assessment and stabilization services - Skill development to reduce or eliminate an individual's symptoms of mental illness so that the person does not need hospitalization. Assist in the transition to a less restrictive placement or living arrangement when the crisis has passed.

DIRECT SERVICE REQUIREMENTS (Required of all employees providing direct services):



  • Promote client independence and growth, consistent with the service plan.
  • Provide services using a trauma-informed and person and/or family-centered model.
  • Provide services utilizing agency approved and supported evidence-based practices that have been indicated by the funder and/or agency.
  • Accurately complete time reporting.



Essential Duties and responsibilities:

FCT SPECIFIC REQUIREMENTS:



  • Works under the direct supervision of the Clinical Director - FCT Supervisor as well as external FCT Foundation Consultant.
  • This role will be expected to work on average, 40+ hours/week on a full time schedule and willing to work non-traditional hours.
  • Work a flexible weekly schedule that may includes frequent evening for client appointments, based on client availability
  • Works within the evidenced-based framework of FCT and the four phase of treatment-Joining and Assessment, Restructuring, Valuing Change, and Generalization.
  • Provides direct service using methods compatible with FCT principles and practices for assigned cases and meets fidelity requirements for treatment intensity (5 hours per week per family).
  • Provides peer support to FCT team and attends FCT weekly team meetings.
  • Conducts FCT joining and assessment activities with the referred individual and their family members, review of referral information, identifying and engaging key participants, identifying systemic strengths and weaknesses, and working with the family on family identified goals.
  • Engages primary caregiver and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
  • Implements conceptualization, treatment planning, intervention, outcome review, and strategy revision procedure using the FCT process.
  • Maintains clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback.
  • Collaborates with all relevant systems and key participants within each system to ensure their buy-in and cooperation throughout FCT treatment.


  • Carries a caseload of 3 to 5 clients, may be more if partnering on cases


  • Significant travel to various locations for meetings, trainings, or to perform responsibilities as needed. Travel will vary depending on case load.


  • Committed to adhering to the FCT principles and model, as well as completing the necessary trainings and assignments to achieve certification as a FCT Clinician within 12 months of hire.
  • Trained/familiar with behavioral, cognitive behavioral, and pragmatic family therapies


  • Provides direct care or face-to-face contact with youth, family and other systems involved with the family (e.g., probation, child welfare, school, employer, etc.)
  • Maintains phone contacts with the family and other systems
  • Maintains clinical documentation, which includes fidelity documentation that must be completed for every client to guide clinical planning, LSS documentation due to clinic status, documentation required for M.A. and/or CCS billing, and in some cases documentation requested by the referral source
  • Participates in supervision and consultation (this occurs for a minimum of 2.5 hours/week)
  • Participates in FCT-specific training, as well as agency trainings
  • Demonstrates flexibility to allow for specific time spent on each responsibility varies depending on intensity of client need during a given week, therapist need for additional supervision, geography, etc.
  • Participates in FCT trainings throughout the calendar year
  • Participates in all additional agency-specific trainings (e.g., HIPAA, cultural competency, etc.).
  • Participates in a team with weekly supervision and consultation
  • Participates in a rotating 24/7 on-call system that has been established by the team

Qualifications

EDUCATION AND/OR EXPERIENCE:

The specific education and/or experience is determined by the service delivered and the funding stream requirements (noted below).



  • Bachelor's degree in a behavioral science or a related field with 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons, or a bachelor's degree in a field other than behavioral sciences with 2,000 hours of supervised post-degree clinical experience with persons with chronic mental illness. Master's degree in human services related field preferred.
  • 2+ years work experience with family systems, trauma, and/or SUD diagnoses preferred.

CERTIFICATES, LICENSES, REGISTRATIONS:



  • Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.



LANGUAGE SKILLS:



  • Ability to read, write, analyze and interpret to complete required documentation by established timeframes.
  • Ability to document in a neat, legible, accurate, objective and consistent manner to reflect the services provided.
  • Ability to effectively present information and respond to questions from all levels of LSS management and employees, as well as the individual, family, and circle of support.



COMPUTER SKILLS/TECHNOLOGY:



  • Working knowledge of computers to allow employee to access computer systems and applications to complete timecards, read and respond to email, and complete required on-line training.
  • Ability to utilize efficiently an electronic health record(s) for documentation of direct services.
  • Must be able to work on computer and utilize computer applications and programs to effectively complete the job.
  • Ability to work within a variety of ever-changing software packages and computer systems.



PHYSICAL DEMANDS/WORK ENVIRONMENT:

TRAVEL: Daily travel may be required and see travel through Sauk, Columbia, Dodge, and Fond Du Lac counties.
