Patient Navigator - Remote within MN, IA, or NE

Pay: $23.20 - $37 per hour (experience will be compensated)

Schedule: Full-time, 40 hours per week

Shift times: Monday - Friday, 8am - 4:30pm

Location: Remote

Job Summary:

Under immediate supervision of the Senior Manager of Patient Access and the general supervision of the Director of Patient Engagement, the Patient Navigator performs a wide variety of functions to facilitate access to abortion services. The Patient Navigator is a critical member of the Patient Services team and actively collaborates with all relevant stakeholders in Health Services - particularly health center leadership and staff. Patient Navigators are advocates in implementing our mission of "Advancing and protecting sexual and reproductive healthcare for all." They may perform other related duties as assigned.

Minimum Qualifications:



  • High school diploma
  • Four years' experience working in a patient/client facing environment
  • Experience working with diverse groups



Your Day-to-Day Responsibilities:



  • In collaboration with the Patient Engagement Leadership Team:



    • Assist in reviewing processes and protocols for abortion patient screening and referrals to navigational support
    • Document, track, and collect data relating to patient referrals to assist the Senior Manager of Patient Access with state and national reports
    • Assist the Senior Manager with reporting as needed.



  • Collaborate with all relevant Patient Services and Health Services teams and individuals (i.e., patient services associates, patient services leadership, health center managers, health center staff, physicians, social workers) to coordinate timely care and access to necessary resources for abortion patients
  • Organize schedules and manage appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on clinical needs and patient preferences.
  • Facilitate communication between the patient, support person(s) and/or organization(s) and healthcare providers to ensure patient satisfaction and quality of care
  • Work with patients to:
  • Facilitate access to financial resources
  • Identify and address logistical needs to access care
  • Determine needed support services and facilitate access
  • Facilitate pre and post abortion visits to ensure continuity of care
  • Facilitate access to resources that will address needs outside the scope of Planned Parenthood
  • Identify and address existing gaps and opportunities within existing patient navigator processes to ensure highest need patients are identified and connected to navigational support
  • Assist in identifying external resources and the evaluation and development of resource guides for patients
  • Work with external stakeholders and organizations to build and strengthen relationships, systems and processes that support access to abortion care
  • Work with external vendors to facilitate access to key services, including but not limited to transportation and lodging
  • Call Handling - Patient Interactions: Registration and Financial Interview

    • Comply with protocols and workflows to accurately collect demographic information for new patients and register them in NextGen
    • Comply with protocols and workflows to verify patient/caller identities for established patients and update demographic info as appropriate
    • Accurately assess patient eligibility for multiple funding sources
    • Collect, enter in NextGen, and verify all required insurance information
    • Educate patients on basic insurance information and practices
    • Collect income and household size; document in NextGen
    • Quote patients a cost for services requested and effectively communicate payment requirements and policies


  • Abortion Scheduling - Abortion Care Services

    • Schedule abortion care appointments for clinics in Minnesota, Iowa and Nebraska
    • Maintain a comprehensive knowledge base on and comply with abortion care laws, regulations and protocols for the four states listed above, including additional laws for minor individuals seeking services
    • Educate patients on the legal requirements for their state
    • Complete basic medical intake and determine which procedure(s) patients are eligible for
    • Explain procedure options to patients
    • Provide funding resources to patients as appropriate
    • Fully prepare patients for their appointment by providing all relevant financial information and appointment instructions
