  1. Oversee and manage all members of the HIV Case Management team which includes management of time and attendance, conducting performance evaluations, and being involved in disciplinary procedures with the assistance of the Director of Care Coordination Programs.
  2. Schedule, design, and conduct department meetings. This includes organizing and tracking any mandatory training that needs to be completed.
  3. Organize the hiring, training, and onboarding for all new hires.
  4. Participate in Quality Management by analyzing clinical and case management data reports periodically to inform the lead implementation of CQI projects in the Ryan White (RW) Program.
  5. Lead the planning and facilitation of RW Consumer Advisory Board (CAB) to present information about current services and quality projects to RW patients and obtain their input to inform the development and implementation processes of those or new projects and services.
  6. Conduct periodic chart audits and work with RW case management team to ensure continuity of care for patients by complying with documentation of program requirements in the EMR accordingly. This may include annual eligibility and enrollment, RW Program required assessments, case management service plans, frequency of medical appointments, labs, screenings, treatments, and referrals.
  7. In coordination with the Director of Care Coordination programs, facilitate the development, implementation, and evaluation of current and new projects within the RW program. Lead multidisciplinary meetings and department meetings.
  8. Participate in the preparation of grant applications and reports.
  9. Represent the department and actively participate in internal interdepartmental or agency-wide and external meetings, committees, training, and projects.


Qualifications:

  • Bilingual (Spanish/English) is preferred but not required.
  • Bachelor's degree in business administration or public health field preferred; consideration will be made for a suitable combination of previous academic, volunteer, and professional experience.
  • Backgrounds working with grants, data collection, and report generation.
  • Competency in Microsoft Word, Excel, and PowerPoint.
