About the Role:

Tidewell Hospice is seeking a Social Workerto support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

• Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
• Provide individual and family counseling, crisis intervention, and bereavement support.
• Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
• Assist with financial resources, Medicare/Medicaid, and long-term care planning.
• Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
• Support patients and families with advance directives and memorial planning.
• Educate the community on hospice care.
• Participate in an on-call rotation.

Qualifications:

• Education & Experience:Minimum one year of social work experience in a healthcare setting (internships may qualify).
• Licensure:Must hold or be in the process of obtaining one of the following Florida licenses within 90 days of hire:
• Licensed Clinical Social Worker (LCSW)
• Licensed Mental Health Counselor (LMHC)
• Licensed Marriage and Family Therapist (LMFT)
• Other Requirements:
• Valid Florida driver's license and reliable transportation for field-based roles.
• Proof of valid auto insurance.

Varied schedule: Provide 64 hours monthly; Monday - Friday 8am - 5 pm. May assist hospice houses, home, ALF or LTC teams.
