Position Summary
This person will provide case management services for up to 15 perinatal clients. The Perinatal Case Manager will assist in providing case management services to Perinatal residents in the residential program. This position will be responsible for assisting the perinatal patients with obtaining Medicaid, TANF support, childcare vouchers, medical appointments and issues, budgeting, post program housing, and other assigned duties to support the families. The Case Manager will participate in multiple treatment teams regarding the families that they are working with. The Perinatal Case Manager also communicates with DSS, CPS, and other external organizations to coordinate necessary care for the families served. This case manager will work closely with the Residential Program manager to ensure all Perinatal beds are full, assisting with obtaining all necessary documentation prior to entering residential treatment. Possibility of case management services on the weekend due to urgent admission or case management duties (unrelated to on-call/crisis services).
Minimum Education and Experience Requirements
Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
Required Qualifications, Competencies, and Experience
Requires a bachelor's degree in a human services discipline (Human Development, Social Work, Psychology, Sociology, etc.), including eligibility of becoming a Qualified Professional status.
Preferred Qualifications, Competencies, and Experience
History, knowledge, and at least one year of exposure to social work or counseling experience; or experience with the substance abuse population, gender-specific treatment, child and family counseling, justice-involved population and a perinatal setting preferred. An ideal candidate will have exceptional organizational skills, time management skills, charting and documentation, and ability to address clinical needs in a timely manner.
In addition, an ideal candidate would have the following experience:
1. Knowledge of SUD in adults
2. Knowledge of record keeping and administrative duties
3. Skills in assessment, case management, advocacy, and individual and group therapy.
4. Knowledge of the needs and concerns of women, including some exposure to issues such as childhood and adult trauma, domestic violence, sexual assault, substance abuse., pregnancy/parenting, and legal and economic difficulties.
5. Ability to work with women from varying incomes, ages, ethnic/racial groups, lifestyles.
6. Knowledge of residential environments
7. Knowledge of supervisory role, especially with paraprofessionals.
