The position typically involves managing a team of transfer coordinators, facilitating communication between various departments, and working closely with physicians, nursing staff, and external healthcare providers. Coordinating patient transfers - Ensuring that patients are transferred to the appropriate facility and nursing unit. Supervise and manage the hospital's transfer center operations to ensure the efficient and timely movement of patients. Coordinate patient transfers between hospitals, clinics, specialty care centers, or other facilities, ensuring appropriate medical protocols are followed. Act as a central point of contact for physicians, hospital staff, and external facilities regarding patient transfers. Collaborating with staff - Collaborate with physicians, nurses, and other medical personnel to ensure that patients being transferred have the necessary medical information and records. Facilitate communication between the transfer center and clinical departments to confirm patient readiness and ensure any required medical equipment is available for transport. Build and maintain relationships with external facilities, including other hospitals, ambulatory care centers, and specialty clinics, to improve coordination and expedite transfer processes. Liaise with insurance companies, emergency medical services (EMS), and other external partners to facilitate transfer logistics and ensure reimbursement for services provided. Ensure Compliance and Quality Standards -Ensure all transfers comply with hospital policies, state regulations, and accreditation standards (e.g., The Joint Commission). Monitor and enforce quality control standards to ensure safe, efficient, and effective transfers that support positive patient outcomes. Adhering to EMTALA guidelines and National Patient Safety Goals Staff Management - Lead and manage a team of transfer coordinators and other support staff. Provide training and continuous education on best practices for patient transfers, ensuring team members are knowledgeable about hospital policies, transport safety, and medical protocols. Monitor team performance, conduct performance evaluations, and provide feedback and coaching to enhance productivity and efficiency. Professional Development - Actively participates in career growth. Develops a plan for learning for self and other including but not limited to customizing and updating education based on current best practices. Participates in projects, as assigned by the Director. Participates on committees/collaborates with committee representatives. Reporting and Data Analysis - Maintain data on transfer operations, including the number of transfers, patient demographics, and any complications or delays encountered.
