We are seeking an experienced Home Health Clinical Manager to oversee and lead the day-to-day operations of our home care and home health services. As a key member of the leadership team, you will be responsible for driving clinical excellence and ensuring the highest standards of care for our patients. About Us: The On Lok Program of All-Inclusive Care for the Elderly at Institute on Aging (On Lok PACE at IOA) is an innovative model of care that is designed to support frail older adults with living- and thriving- in the community for as long as possible. We serve as a primary care team and a health insurance plan, which allows our interdisciplinary team of healthcare professionals to coordinate the right care at the right time, and to meet each patient's unique needs. Home care is a required discipline on our eleven-member interdisciplinary team. The home care department is responsible for providing personalized, in-home care that supports patients' independence and improves their overall health and quality of life. This ensures that care at home is not just well-coordinated but is an integral part of the interdisciplinary team's care plan. Role Overview: As the Home Health Clinical Manager, you will be responsible for overseeing the clinical operations and ensuring the effective delivery of home care and home health services. You will lead a dedicated team of nurses and caregivers guiding them in best practices while ensuring compliance with regulatory standards. Your leadership will ensure that our patients receive the highest level of care, all while optimizing operational efficiency. Key Responsibilities:
