DESCRIPTION OF WORK

This is a trainee position (probationary period of six months) within the Luzerne/Wyoming Counties Area Agency on Aging. This employee is specifically trained to assess the needs of consumers utilizing state mandated assessment forms, develop effective care plans to meet needs, and implement care plans in a professional manner.

You will be responsible for completing Reports of Need on consumers aged 18 and older, after successful completion of required training courses. You will have the opportunity to make priority home visits for consumer safety and work as a liaison between the consumer and provider.

Other duties to include but not limited to:

  • Assisting consumers, guardians, and families in maximizing their abilities for self determination by enabling them and empowering them in decision making to the greatest extent possible
  • Reassessing consumers on an annual or biannual basis
  • Identifying changes in situation or function and measure progress toward the goals
  • Educating consumers and their families regarding aging services and benefits.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Three years as an Aging Case Aide 2; or
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

  • Hiring preference for this vacancy may be given to candidates who live within Luzerne/Wyoming Counties. If no eligible candidates who live within Luzerne/ Wyoming Counties apply for this position, candidates who reside in Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, or Sullivan Counties will be considered. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
