Come talk to us about joining the Centerstone team! JOB DESCRIPTION: SUMMARY OF POSITION The Older Adult Project Coordinator provides clinical services to adults age 50+. The PC has the opportunity to provide individualized needs assessment, and to tailor individualized care plans for service recipients. Services may include intake assessment, diagnosis, individual therapy, care management, linkage, advocacy, psychosocial groups, and caregiver support. The PC also connects with community stakeholders to identify needs, provide education, and participate in strategic partnerships to expand service delivery for Older Adults. The PC will coordinate with the TN Department of Mental Health and Substance Abuse via monthly reporting and quarterly partnership meetings. The PC will provide outreach and educational services in the community. No weekend or on call work required. Licensure supervision may be available. ESSENTIAL DUTIES AND RESPONSIBILITIES
