RN Research Care Coordinator (Temporary Casual)

Duties and Responsibilities:

Position Summary:
The RN Research Care Coordinator is responsible for overseeing and coordinating the healthcare needs of individuals who participate in a study related to Alzheimer's disease testing in the American Indian (AI) population at the University of Minnesota. Individuals who participate in this research study are provided with an assessment and recommendations for follow-up care. This role involves working collaboratively with a multidisciplinary team to ensure that participants receive comprehensive, coordinated care that is aligned with their individual health goals as well as the recommendations provided by the research team. The RN Research Care Coordinator will assist with coordinating follow-up for patients with urgent medical needs, connecting participants to primary care providers if they do not have ongoing care, and arranging for specialist visits when medically necessary. The RN Research Care Coordinator plays a critical role in enhancing participant outcomes, optimizing resource utilization, and improving research participant satisfaction.

Key responsibilities:

* Patient Assessment and Care Planning
o Conducts comprehensive assessments of participant needs including access to care, health status, physical, psychosocial, and environmental factors.
o Develops individualized care plans in collaboration with research participants, their caregivers, and the research team.

* Care Coordination:
o Coordinate care across various settings, including primary, specialty, and community-based services.
o Facilitate communication among healthcare providers, participants, and families to ensure seamless transitions and continuity of care.
o Assist with coordination of care directly related to the research study including transportation and provider visits.

* Patient Education and Advocacy
o Provide education to patients and families about disease management, treatment plans, and healthy lifestyle choices.
o Serves as a resource for questions related to test results, imaging results, and recommendations from the research team providers.
o Serve as an advocate for the specialized needs of the American Indian population ensuring adequate follow-up and care is attainable.
* Resource Management
o Assist participants in accessing community resources and support services when necessary.
o Use culturally competent care to advocate for participants' needs and preferences, ensuring they are central to the care planning process.

* Documentation and Reporting:
o Maintain accurate and up-to-date patient records, ensuring compliance with regulatory and organizational standards.
o Prepare reports and participate in quality improvement initiatives.

All required qualifications must be included in the application materials

Required Qualifications:
o Current Registered Nurse (RN) license in good standing in the state of Minnesota
o Certification in Care Coordination or Case Management is a plus, but not required.
o Minimum of 4 years of clinical nursing experience.
o Experience working with underserved populations preferred.
o Excellent communication and interpersonal skills.
o Strong organizational and problem-solving abilities.
o Ability to work independently and as part of a team.
o Proficiency in using electronic health records (EHR) systems.

This position is located in the Department of Neurology.

Pay Range: $50.00 - $67.00 per hour; depending on education/qualifications/experience.
