Job Description

The Center for Behavioral Health, located at 12837 Flushing Meadows Dr. Ste 220, Town & Country MO 63131, seeks an enthusiastic Patient Navigator to join our team. UMSL - Center for Behavioral Health is amongst the largest psychological assessment practices in Missouri and is considered the primary provider of psychological evaluation services in the St. Louis region. For over 40 years, we have provided comprehensive, high-quality assessments to some of the most underserved members of our community. The Center maintains strong referral relationships with more than 60 community partners, including state agencies, school districts, outpatient and inpatient mental health facilities, and primary care providers. As a training clinic at UMSL, CBH remains at the forefront of assessment practice and our staff regularly benefit from our extensive ongoing professional development program.
Requires computer proficiency in Microsoft Office programs, the ability to implement pre-determined processes accurately and consistently with strong attention to detail, and the ability to establish and maintain effective professional relations with clients, co-workers, and other agencies. The successful applicant should be flexible, organized, and comfortable working with the public.

Duties and responsibilities include:

  • To serve as patient navigator, liaising between clients, caregivers, healthcare providers and other social service agencies to facilitate communication among treatment team members.
  • To gather information from clients and caregivers served by UMSL - Center for Behavioral Health.
  • To provide support and direction to clients and their caregivers.
  • Interview clients, caregivers, and community members to identify case management needs of clients receiving psychological evaluation services.
  • Assist treatment team and caregivers with locating needed resources and/or support.
  • Refer clients and their families to other agencies for additional and/or different services and collaborate with other agencies to obtain needed services.
  • Follow up with clients and caregivers as necessary throughout treatment and after discharge.
  • Maintain appropriate documentation and outcome databases.
  • Coordinate and participate in the clinic's outreach activities.
  • Serve as a backup to CBH's intake staff as needed: performing telephone and walk-in intakes with prospective clients, attending staff meetings, preparing correspondence, and providing office support to clinical staff.
  • Other administrative duties as assigned.

Shift

M-F; 40 hours/week

Minimum Qualifications

High school diploma or equivalent and at least 1 year of experience from which comparable knowledge, skills and abilities can be acquired is necessary. Certifications as required by regulatory agencies is necessary

Preferred Qualifications

Bachelor's Degree in related field

Anticipated Hiring Range

Pay commensurate with education and experience.
Hiring Range: $17.50 per hour
Grade: GGS-006
University Title: CLINICAL TECHNICIAN II
