Job Summary: The Program Manager-Sickle Cell Program (MSW) is experienced with a solid body of knowledge related to the specialty practice of sickle cell disease management, treatment, and services. Reports to the Director-Sickle Cell Services. Keeps pace with a proactive and progressive management team and effectively coordinates the sickle cell program and related projects. Participates in strategic planning for the sickle cell program. Coordinates with the Sickle Cell Center program director, medical director, administration and Patient Safety and Reliability (PSR) to achieve and maintain a high level of care to patients served at the Sickle Cell Center. Functions as a liaison between patients, families, clinics (internal and external referring clinics), hospital in-patient units, and support services. Utilizes discretion and independent judgement to apply care coordination methodologies and practices for a seamless multidisciplinary approach to whole-person care across the continuum. The Program Manager also acts as a social worker and conducts psychosocial assessments to assist with care coordination. Aids patients and their families in the form of diagnostic evaluation, individual, family & group therapeutic counseling and resource referral. Refers clients to community resources to enhance compliance with a complex long-term medical regimen. These services may be provided for both the LLUMC and LLUCH hospitals. Develops and maintains knowledge of referral resources throughout the service area. Performs the assessment process independently and provides direct and indirect age-specific and developmentally appropriate support to high-risk and complex patients with sickle cell disease and multiple comorbidities. Applies theoretical concepts to social work and care coordination practice and exhibits self-motivation to continually expand knowledge base and skills with a focus in sickle cell disease. Serves as a resource and a liaison among physicians, staff, patients, families, and payors to ensure effective utilization of resources and optimal care coordination. Participates in developing, implementing, and maintaining policies, procedures, protocols, documentation templates, that are parallel with best practices for all aspects of care coordination related to sickle cell disease management. Collaborates with the director in budget preparation, compliance, and program development, implementation, and evaluation. Participates in local and national activities and initiatives to provide educational experiences for healthcare providers, patients, and the community. Develops networks and alliances that promote the sickle cell program while augmenting visibility and enhancing the sickle cell service image. Identifies opportunities for community outreach programs that promote LLUH as a regional resource and center of excellence for sickle cell disease management. Serves as a chair in the LLUH Sickle Cell Center program community boards. Leads and manages community outreach program activities, events, and budgets. Contributes to planning seminars, conferences, and symposiums for educational events related to sickle cell disease. Works collaboratively with the director to maintain program grant and funding, including monthly progress and financial reports. Assists the director with managing grant workplans and budget tracking. In collaboration with the director, the program manager will also participate in grant funder site meetings. Serves in all stakeholder meetings as a representative of the LLUH Sickle Cell Center program. Demonstrates behaviors which supports excellence in customer satisfaction, patient safety and professionalism. Demonstrates expertise in service recovery methodologies and capably resolves issues from dissatisfied customers. Contribute to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.
