Dementia Care Program Manager

The Gilchrist Dementia Care Program Manager is responsible for the development, coordination, implementation, education and administration of programmatic duties to include planning and organizing the program, interfacing with internal and external stakeholders, and driving excellence in dementia care across Gilchrist programs. The program manager assures that the organization meets the performance metrics of the program as outlined by CMS and other funding sources.

Education

Master of Social Work degree from a university accredited by the Council on Social Work Education

Experience

Minimum of 2-3 years Social Work experience required. Experience in medical social work or hospice in multiple care settings preferred. At least two years of management experience in a healthcare setting preferred.

Licensures, Certifications

  • Licensed in the State of Maryland by the Board of Social Work Examiners as an LCSW-C (Certified Social Worker - Clinical), or LCSW (Certified Social Worker). Valid driver's license and safe driving record and an automobile that is insured in accordance with Gilchrist requirements

Physical Requirements

  • Ability to travel to patient locations; includes ability to drive, negotiate stairs and walkways, including uneven surfaces at private homes or facilities
  • Ability to utilize electronic record keeping systems, computers/keyboards and or laptops and other related equipment
  • Ability to communicate verbally in order to assess and impart information concerning Gilchrist's services and programs as well as with team and external providers

Working Conditions

  • Driving as large part of the day in the field, which includes visits to patient's homes, Independent and Assisted Living Facilities, throughout Gilchrist service area. Exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases

Patient Population

  • Demonstrates competency in the delivery of care and applies the knowledge to meet age-specific needs if applicable.

Principal Duties and Responsibilities

  • Collaborates with local, state, and federal partners regarding dementia care programmatic opportunities and implementation.
  • Assists with the development and submission of all program applications for funding.
  • Assists with the development and manages the department budget.
  • Recruits and manages the staff for the care and treatment of dementia patients for various programs.
  • Adheres to existing policies and procedures.
  • Develops, plans and coordinates patient, staff, volunteer and community education regarding dementia care. Designs discipline-specific training tracks and modules for Gilchrist support staff and clinicians to complete and track compliance to payer program requirements.
  • Curates and develops resources on an ongoing basis to support internal and external care partners, to include patient families and other healthcare entities.
  • Maintains up-to-date knowledge of the CMS Innovation and other payor models for the service delivery and eligibility requirements for patient alignment and communicates these requirements to all team members. Assures the organizations compliance with all program requirements and productivity standards.
  • Participates as a member of the Interdisciplinary Team which delivers specialized dementia care to patients and their families in their homes regarding completion and execution of care plan as outlined by various funding sources.
  • Meets with patients and families to provide education regarding our dementia programs, address questions and concerns, facilitate timely referrals and enrollment as desired by patient/beneficiary.
  • Coordinates completion and submission of all health and administrative documentation in line with all regulatory compliance requirements.
  • Provides one-on-one and group consultations to care partners (internal and external) caring for people living with dementia.
  • Manages collection and reporting of beneficiary-reported sociodemographic data and health-related social needs (HRSN) data from patients/beneficiaries.
  • Designs process to facilitate and track environmental modification for Gilchrist patients living with dementia.
  • Designs and facilitates workflow for respite benefit, to include referrals, settings of care, and documentation.
  • Collaborates with referral sources and Gilchrist colleagues to facilitate seamless transitions to and from other Gilchrist services.
  • Coordinates with EPIC team to streamline workflow for documentation across disciplines.
  • Works with data team to develop dashboards to monitor and measure the performance of all dementia care programs.

Pay Range

$75,318.06 - $135,572.50

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
