Under general supervision, provides psycho-social services including psychosocial assessment and interventions, discharge planning, coordination of services, and resource referral.

Education

Master's Degree in Social Work; Preferred Clinical track.

Experience

3 years of clinical social work experience, preferably in a medical setting

Knowledge, Skills and Abilities

  • Knowledge of various social, home care, extended care, hospice, hospital government program, commercial insurance, and community services
  • Knowledge of social work theories, therapies, and techniques of casework process
  • Knowledge of principles and regulations of public welfare and Medicaid
  • Skill in coordinating efforts of a multi-disciplinary team (agency, hospital, hospice, payer, etc.)
  • Skill in needs analysis
  • Ability to develop associated treatment modalities
  • Customer service skills
  • Skill in word processing programs
  • Skilled in grief and bereavement
  • Able to work independently with minimal oversight

Licensures, Certifications

  • LCSW-C preferred
  • Advanced Palliative Hospice Social Worker Certified (APHSW-C , preferred

Physical Requirements

  • Ability to travel by car to homes and navigate different home settings.

Working Conditions

  • Work is performed inside the organization's offices and patient/family homes. Job attendance is required during all types of weather conditions.

Conditions of Employment

  • Must speak, write, and understand English fluently both in person and on the phone.
  • Adherence to National Association of Social Workers (NASW) Code of Ethics and NASW Standards for End-of-Life Care
  • Maintain current license to practice as a professional social worker
  • CAPC competency completion within 180 days of employment 90 or 180 days??

Patient & Workplace Safety:

Patient Population:

  • Demonstrates competency in the delivery of care and applies the knowledge to meet age-specific needs if applicable.

Principal Duties and Responsibilities

  • Consults and follows patients in a community based Palliative Medicine program as assigned. (Let's leave the option open for facility work as well.
  • Meets with family members and significant others to obtain a relevant psycho-social assessment and to assess current and potential medical and social drivers of health needs.
  • Develops and implements a plan of care appropriate to identified problems. Shares resultant findings with the appropriate health care team members. Care plans will meet the patients and family's care goals and minimize burdensome care transitions and uncomfortable symptoms of their disease. Suggests and arranges for hospice care when appropriate.
  • Provides counseling services including individual, family and group modalities and other recognized psycho-social therapies in assisting patients and family members.
  • Provides interpretation of the patient's, significant others, and family member's behavior as needed. Offers suggestions for modifying behavior to enhance a therapeutic approach.
  • Provides and facilitates timely completion of appropriate documentation for patient records.
  • Assists team to coordinate referrals, appropriate levels of care, and the use of therapeutic community resources to ensure a safe and appropriate care plan as well as discharge plans.
  • Provides as well as attends educational in-services for community organizations and providers on palliative and hospice care.
  • Participates actively in problem solving to enhance the most efficient and appropriate plan of care.
  • Coordinates with other Gilchrist Programs to build/develop a complete Palliative Medicine service line. ?? Participates in palliative medicine program development in collaboration with the palliative medicine interdisciplinary team and other service lines.

Pay Range

$51,351.55 - $79,594.90

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
