Responsibilities

The LVN Discharge Planner is responsible for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker in the development and implementation of the discharge plan. This role will be traveling to local Prime Healthcare hospitals to support their needs and vacancies.

The LVN Discharge Planner will be trained by our corporate Case Management Department for the first three months upon hire (Ontario, CA) before being assigned to one of our local Los Angeles-based facilities (Inglewood, CA or Lynwood, CA) where they will be based out of going forward. They will maintain and coordinate Medicaid state submission forms for presentation to on-site reviewer and files them accordingly or as per facility's procedure. Assists with case management referrals, discharge planning as assigned, by interviewing patients/families, follow-up on required assignments and organizing the development of a safe patient-focused discharge plan.

Qualifications

Required qualifications:

    Associates degree or LVN license required.
  1. Travel to local hospitals is required.
  2. Knowledge in discharge planning/utilization management/case management terminology and functions, in both managed care and non-managed care environments.
  3. Experience in basic to intermediate computer skills/knowledge.

Preferred qualifications:

  1. Current BCLS certificate required upon hire and maintain current preferred.
  2. Experience in the hospital setting preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $32.00 to $37.00. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
