Position Details As a member of our Intensive Transition Services (ITS) Team, you'll play a vital role in delivering clinical therapeutic interventions, care coordination, and supportive services in a collaborative, multi-disciplinary environment. * Provide recovery based, person centered services * Develop comprehensive goal plans in conjunction with individuals and family members. * Provide Service coordination, including coordinating and monitoring the activities of the individual's treatment team. * Develop, write, implement, evaluate and revise, as necessary, individual's treatment plans in collaboration with the Treatment Team. * Provide individual and group supportive interventions and symptom management ensuring that immediate changes in the treatment plan are made, as necessary. * Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process. * Educate and support individual's families and advocate for individual's rights and preferences. The ideal candidate will possess the following qualifications: * Bachelor's degree in Social Work, Psychology, or another human service related fields * One to two years of experience in the behavioral health field working with individuals with development disabilities. * One year of experience in the provision of case coordination and case management services preferred * A personal vehicle and drivers license.
