LSSI is hiring a full-timeCCS Intake Coordinator, Monday through Friday. Responsible for responding to all referrals from the CCS Case Work team (in-person & telephonic) and completes initial screening for new clients as well as scheduling initial intake appointments with the appropriate programs.

Essential Functions:
  • Respond to all referrals from the CCS Case Work Team.
  • Refer clients that do not meet criteria for LSSI services to care coordination for evaluation and Image to additional resources.
  • Verify benefits and match insurance coverage to the appropriate service or provider.
  • Maintain knowledge of LSSI programs and alternative community resources.
  • Manage influx of new clients and program size.
  • Schedule intakes and referral assignments with appropriate counselor.
  • Utilize Electronic Health Record and client tracking systems.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
Position Qualifications:
In addition to a real passion for working with at-risk children and families, this opportunity requires:
  • Associates degree in counseling, social work or related human service field required.
  • Background check clearance required.
  • Demonstrated proficiency using electronic health record and client tracking systems.
  • Experience with intake assessment and scheduling client appointments preferred.
  • Demonstrated ability to communicate in a clear, comprehensible manner, bot verbally and in writing.
  • Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Valid driver's license, in good standing for the state of residency required.
  • Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
