Overview

Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for a Health Educator. The Health Educator is responsible for teaching people about behaviors that promote wellness and the case management of the learning experiences that provide individuals the opportunity to make appropriate health decisions. The health educator will develop and implement appropriate strategies and methods to facilitate health promotion among those individuals and the community.

This a part-time (12 hrs/week) grant funded position that will be based out of Our Place, a homeless services drop-in center in West Oakland. The Health Educator will provide support to Our Place clients to access hygiene services like showers, laundry, and supplies. The Health Educator will also distribute COVID-19 rapid tests and educate clients about COVID-19 risk mitigation strategies, as well as help provide clients supplies to safely isolate or quarantine in the community.

This position is grant funded until March 31, 2025.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Compensation: $22 - $26/hour.

COVID-19 Vaccine Policy

In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Unless a reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.



Responsibilities

    Develop and assist in the development of health education materials ie. Curriculum, workshops, bulletin board, and flyers for both in person and virtually. Materials need to be culturally sensitive and tailored to the unique needs of our communities.
  • Facilitate individual and group health education sessions.
  • Teach patients how to cope with or manage existing health conditions.
  • Coordinate and implement health education campaigns.
  • Provide follow up health education per medical, dental and/or behavioral health provider request.
  • Document face to face and phone calls within patient charts in an Electronic Health Record system when necessary.
  • Connect patients and families to health services or information.
  • Communicate and advocate for improved health resources and policies that promote health.
  • Maintain all health education materials and resources.
  • Evaluate the effectiveness of programs and educational materials.
  • Assist in patient flow by checking in/ checking out patients, answering telephones, providing basic first aid.
  • Perform other duties as assigned by supervisor.


Qualifications

  • Experience working with youth, working with community outreach techniques, and a demonstrated ability to work in a multi-cultural setting and with diverse populations.
  • Demonstrated knowledge of principles, methods and materials used in health education.
  • Demonstrated knowledge of current comprehensive family planning and STD prevention methods.
  • Ability to prioritize competing work demands and tasks from clients or staff
  • Ability to work effectively and calmly under pressure in a positive, friendly manner
  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
  • Ability to seek direction/approval from on essential matters, yet work independently, using professional judgment and diplomacy.
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
  • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
  • Make appropriate use of knowledge/ expertise/ connections of other staff.
  • Be creative and mature with a "can do", proactive attitude and an ability to continuously "scan" the environment, identifying and taking advantage of opportunities for improvement.

Job Requirements

  • High school diploma/equivalent
  • Minimum of one year of experience in a community clinic or medical office setting.
  • Ability to 'float' to various SHC sites depending on need
  • Excellent interpersonal skills, verbal and written communication skills and computer skills are essential.
  • Group facilitation, presentation and organizational skills.
  • Proficient in Microsoft Office and the internet.
  • Current CPR from AHA approved organization.

Job Preferences

  • Bachelor's Degree in health education or related field.
  • Electronic Health Records and Electronic Practice Management systems experience
  • Working knowledge of community health problems including social and economic factors relating to health.
