Rehabilitation Admission Coordinator

JOB DESCRIPTION:

Working within a team environment, evaluates patients and makes decisions for Inpatient Rehabilitation admission as appropriate. Communicates with patients, families, physicians, acute care staff, Inpatient Rehabilitation staff, other team members and insurance representatives as necessary throuout the process. Maintains collaborative relationship with physiatry consult service, Inpatient Rehabilitation staff and other team members. Educates patients, families, health care providers, insurance representatives and community about Rehabilitation Center Programs.

EDUCATION:

State of Vermont Registered Nurse (RN) license required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative; CRRN preferred.

EXPERIENCE:

3-5 years, work in rehabilitation program or with patients who need rehabilitation.
