Bloomington Corps

Position Title: Case Manager

Job Summary: Provide case management services to clients to include, but not limited to: reviewing intake material for accuracy, assessing client needs, case plan development and compliance, act as a liaison to community resources, provide referrals to appropriate agencies, arrange for client transportation

Report To: Residential Services Director

Supervises: N/A

Qualifications:



  • Candidates must have a passion for fulfilling the mission of The Salvation Army
  • Candidates must successfully pass the pre-employment background inquiry and annual motor vehicle check with a valid driver's license



Education:



  • Bachelor of Science degree in social work, sociology, criminal justice or other related field. In lieu of a degree, candidates with a minimum of five years' experience in social work or case management with a social service organization will be considered.



Skills:



  • Computer proficiency in: Microsoft Word, Excel, and HMIS Service Point is highly desired along with
  • Excellent oral and written communication skills;
  • Familiarity with local resources, Continuum of Care and homeless service providers;
  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio economic backgrounds in a sensitive and culturally appropriate manner

Essential Responsibilities:



  • Provide case management services to clients to include, but not limited to: reviewing intake material for accuracy, assessing client needs, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, arrange for client transportation
  • Insure that all clients have access to a case manager through appointments and walk ins


    • Establish first contact within 5 days
    • Meet with clients weekly or as otherwise determined
    • Attempt to meet for discharge planning


  • Document client information and progress through HMIS Service Point.
  • Record, track and submit statistical information as scheduled to maintain compliance with DHS, HUD, United Way and The Salvation Army.
  • Attend monthly Continuum of Care (COC) meetings as directed
  • Conduct quarterly resident house meetings as directed
  • Determine client eligibility.
  • Act as an advocate for clients when needed
  • Act as first point of contact for client related issues to include:


    • Addressing disputed incident reports
    • Render appropriate resolution with flexibility and discretion


  • Maintain all client records ethically and confidentially.
  • Strive to meet the outcomes of the Continuum of Care grant project.
  • Attend conferences, seminars and staff meetings as directed.
  • In administering Safe Harbor programs, uphold the policies, goals and mission statement of The Salvation Army.
  • Collaborate with outreach workers and maintain eligibility list for non-residents utilizing Day Center Services.
  • Facilitate outreach and training activities for clients by maintaining community partnerships.
  • Participates in rotating ON CALL schedule to provide 24/7 coverage to Safe Harbor operations.
  • Develop Job Leads Listing when Employment Specialist is unavailable
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
  • Maintain compliance in The Salvation Army's "Safe From Harm" Certification.
  • Other duties as assigned by the Residential Services Director or Corps Officer



Position Ranking:

Regular Full Time, Non-Exempt, Pay Range $22.00 to $24.00 per Hour

Physical Demands/ Work Environment:

This Position is required to do light to moderate physical work. In order to successfully perform the essential functions of this position, the individual is regularly required to remain stationary for some periods of time, use hands and fingers, communicate, move, occasionally required to position self to stoop, kneel or crouch, frequently required to reach with and use arms and hands as required in using standard office equipment and a PC. Specific vision abilities by this job include close vision, distance vision and ability to focus. The work environment for this position includes an office environment with a low noise level.
